Objective Assessment of tranexamic acid impact on visible andespecially hidden blood loss after unilateral total hip arthroplasty.Methods We chose60patients (male18, female42), age58-86years (average72.58years), who received primary unilateral total hiparthroplasty due to femoral neck fracture. Randomly divided them intothree groups: Group A (n=20): No tranexamic acid, We just gave thepatients100ml NS when started the operation; Group B (n=20):Intraoperative tranexamic acid, We gave the patients20mg/kg tranexamicacid within100ml NS when started to cut the skin; Group C (n=20):Preoperative tranexamic acid,1hour before surgery we gave the patients20mg/kg tranexamic acid within100ml NS and We started the operationabout half a hour after the intravenous infusion of the drug completely. Wemeasured the total red blood cell loss, visible red blood cell loss and hiddenred blood cell loss of all the three groups, and carried out the comparativeanalysis. Results The visible red blood loss in group A,group B and group Cis83.73ml±15.08ml,45.47ml±11.48ml and45.28ml±8.35mlrespectively; the hidden red blood loss in group A, group B and group C is334.08ml±57.69ml,203.81±46.57ml and137.60±34.69ml respectively.The visible and hidden red blood red cell loss in group B are bothsignificantly less than that in group A (P<0.05). Although the visible redblood red cell loss in group B and group C are without statisticallysignificant difference(P>0.05); the hidden red blood red cell loss in groupC is significantly less than that in group B(P<0.05).Conclusion tranexamic acid reduces both the visible and hiddenblood loss after the primary unilateral total hip arthroplasty, and maybe1hour preoperative use of tranexamic acid is more effective on the hiddenblood loss than intraoperative use.
