Purpose:To investigate the degree of inter-arm blood pressure difference(IAD)and the detection rate of abnormal IAD in patients with hypertension;to analyze the influencing factors of abnormal IAD in patients with essential hypertension;to explore the bilateral arms of patients with hypertension There is a correlation between the difference of blood pressure and the type of medical syndrome,which provides a theoretical basis for the diagnosis and treatment of essential hypertension by integrated traditional Chinese and Western medicine.Methods:Collect information on inpatients from the Department of Hypertension and Sleep Disorders and Cardiology of the Affiliated Hospital of Gansu University of Traditional Chinese Medicine from June 2017 to December 2018.Collect general demographic data,laboratory data,and pairs of 400 patients who meet the criteria.The upper arm blood pressure,arteriosclerosis related indicators,clinical symptoms and other indicators,the patients in accordance with the standard according to the "vertigo disease(primary hypertension)TCM clinical pathway implementation program(trial)in the syndrome diagnostic criteria are divided into Yin deficiency Yangshuo The syndrome,liver qi stagnation certificate,phlegm and blood stasis syndrome,yin and yang deficiency syndrome type 4,the collected clinical information and physical and chemical examination results were entered into the Excel table,using SPSS21.0 software for statistical analysis of data.Results:1.A total of 400 patients with essential hypertension who met the criteria were included in the study.The mean age of the patients was(61.72±11.62)years.Among them,268 people took antihypertensive drugs,accounting for 67.00% of the total;109 cases of smoking,accounting for 27.25% of the total;143 cases of sleep disorders,accounting for 27.25% of the total.2.The mean right systolic/diastolic blood pressure of the 400 patients with essential hypertension selected in this study was 163.36±19.46/86.12±13.25 mmHg,and the mean systolic/diastolic pressure of the left arm was 160.47±18.00/86.23±12.91 mm.Hg,right arm blood pressure is slightly higher than the left arm.The absolute difference between systolic and diastolic pressures between the arms was 5.99 ± 4.41/3.19 ± 2.28.3.400 cases of patients with essential hypertension were classified into 4 types according to their syndrome differentiation.The distribution of the syndrome types from high to low was: 125 cases(31.25%)of yin deficiency and impotence syndrome.There were 104 cases of mutual syndrome,98 cases of liver qi stagnation syndrome(24.50%),and 73 cases of yin and yang deficiency syndrome(18.25%).4.In this study,400 patients were included,the detection rate of s IAD?10mmHg was17.25%(69/400),and the detection rate of d IAD ? 10 mm Hg was 1.5%(6/400).The detection rate of s IAD?10mmHg in female patients was 17.59%;the detection rate of d IAD?10mm Hg was 0.46%;the detection rate of IAD?10mmHg in male patients was 16.85%;d IAD?10mm Hg The detection rate was 2.72%.There was significant difference in ABI between different syndromes in patients with essential hypertension(P<0.05).The difference between the two groups was statistically significant(P<0.05).),and significantly lower than the other three types of syndromes.The difference of ba PWV between different syndromes was statistically significant(P<0.05).After comparison,the differences between the two groups were statistically significant(P<0.05),and significantly higher than the other syndromes.The other three types of syndromes.The difference of s IAD between different syndromes in patients with essential hypertension was statistically significant(P<0.05).After comparison,the differences between sputum and stagnation syndrome were statistically significant compared with other syndromes.<0.05),and significantly higher than the other three types of syndromes.There was no significant difference in d IAD between different syndrome types(P>0.05).7.Abnormal s IAD related factors univariate analysis showed that: s IAD normal group and abnormal group of patients with age,body type,blood pressure grading,takingantihypertensive drugs ratio,ABI value,a total of 5 indicators were statistically significant The significance(P<0.05),and the gender ratio,fasting blood glucose abnormality ratio,sleep quality,smoking rate,TG value,TC value,LDL value,Ba PWV value of the two groups were not statistically significant(P >0.05).The results of binary logistic regression analysis showed that there was a significant positive correlation between blood pressure level and sIAD in patients with hypertension(P<0.05).There was a significant negative correlation between ABI and sIAD in patients with hypertension(P<0.05).8.Abnormal d IAD univariate analysis showed that the sex ratio,age distribution,body shape,blood pressure level distribution,taking antihypertensive drugs,fasting blood glucose,sleep,smoking history,TC in the normal and abnormal groups of d IAD group There were no significant differences in TG,LDL,ABI,and Ba PWV(P>0.05).Therefore,no further multivariate logistic regression analysis was performed in this study.Conclusion:1.There is a difference in blood pressure between the arms of patients with essential hypertension.It is very important to accurately measure the blood pressure of the upper limbs during clinical diagnosis and treatment.2.The results of this study show that yin deficiency and impotence syndrome and phlegm and blood stasis syndrome are the most common syndromes in patients with essential hypertension.3.The difference of s IAD between different syndromes in patients with essential hypertension was statistically significant compared with the other syndromes,and was significantly higher than the other three syndromes.There was no significant difference in the d IAD index between the four syndrome types.4.Abnormal s IAD is associated with low ankle brachial index,blood pressure grade and abnormal s IAD.
