Objective:To understand their’s awarenesses of hypertension knowledge andthose factors which affected blood pressure control by launching the fieldepidemiology investigation to elderly patients with essential hypertension fromuniversities and collages in Nanchang city of Jiangxi province; then the bloodpressure control of these factors in different levels were analyzed by decision tree, inorder to provide the basis or reference for the management of hypertension andrelated chronic diseases from communities.Methods:(1)The material and information of fourteen provincial undergraduateuniversities or colleges in Nanchang city were obtained with the support of the teacharts committee of Chinese People’s Political Consultative Conference (CPPCC) inJiangxi province. The draw-out colleges/universities and objects were to sure bytwo-stage sampling method. Firstly, five colleges and universities were extracted asfield sites by simple random sampling method. Secondly, elderly patients withessential hypertension from each site that fit the requirements for this study wereuniformly numbered, afterwards the objects was ensured by the method of therandomized digital table.(2)Field investingation methods:①Filling in the Questionnaire After theparticipants concentrated, contents of the questionnaire were explained and questionswere answered by the expert. The questionnaires were truthfully filled in according totheir own conditions under investigators’ supervision and guidance. Meanwhile, thequestionnaires were completed by investigators using the face to face way for thoseobjects who were completely unable to fill.②Physical Measurement Bloodpressure, height, weight, waist circumference of the objects were measured.(3)The database was set up and data were double entered by EpiData3.1and thedata were analyzed by SPSS13.0. The statistical description mainly used absolutenumber and constituent ratio; the statistical inference used corresponding statisticalmethods based on data nature. The C5.0algorithm of of Clementine12.0was used tomake decisions for influencing factors of blood pressure control or not. P values were bilateral inspection results and the inspection level of alpha was0.05.Results:(1)In this study,5collages were selected; they are Nanchang University,Jiangxi normal University, Nanchang aviation University, Jiangxi agriculturalUniversity and Jiangxi College of traditional Chinese medicine.480copies ofquestionnaires were issued in the survey and433copies of effective questionnaireswere taken back, the effective rate was90.2%. According to effective researchinformation, the proportion of hypertension grade1and2was more, up to77.5%; theproportion of non-ISH was62.9%, and ISH was37.1%.(2)The full awareness rate of hypertension related knowledge among elderlypatients with essential hypertension from universities and collages was low, only8.3%; the part of awareness rate was high, up to84.1%. Differences were not obviousin different gender and different age groups. In the degree of education, the awarenessrate of objects who were in junior middle school and the following was lowest(64.3%); in the aspect of economic income, it was lowest in below RMB2000yuanof average monthly income (79.1%).(3)The blood pressure control rate of elderly patients with essential hypertensionwas48.7%. There were no differences in genders, age groups, degrees of educationand economic income.(4) Single factor analysis demonstrated that there were12factors which affectedblood pressure control in patients, they were disease course, hypertensionclassification, ISH or not, family history of hypertension or not, number ofamalgamative disease, regular medication or not, antihypertensive measures, time ofsmoking cessation, amount of cooking oil and salt, physical exercise or not andwhether the emotion was stable.(5)In the decision tree model built by C5.0algorithm of Clementine12.0, theclassified accuracy of training samples was84.35%, Kappa value was0.74; andtesting samples was83.45%,0.70, respectively. From the tree graph, the variable ofnumber of amalgamative disease was the most critical factor that affected bloodpressure control; then antihypertensive measures, physical exercise or not,hypertension classification, regular medication or not, whether the emotion was stable,amount of cooking salt were followings. Conclusions:(1)Among the elderly patients aged60years old with essentialhypertension from universities and collages in Nanchang city, the proportion ofhypertension grade1and2was more, most in mild to moderate levels; the proportionof ISH was a third of all elderly patients with essential hypertension. The fullawareness rate of hypertension related knowledge among objects was at low level.(2) The total blood pressure control rate of elderly patients with essentialhypertension was48.7%, it was obvious higer than general population, but lower thansome communities in developed cities. Another50.0%or more were out of control,the result prompted that it would go a long way toward work of blood pressurecontrol.(3)There were12factors from related significant influencing factors thataffected blood pressure control of elderly patients with essential hypertension, andther were7factors that entered the decision tree model. They were number ofamalgamative disease, antihypertensive measures, physical exercise or not,hypertension classification, regular medication or not, whether the emotion was stable,amount of cooking salt. The accuracy of the decision tree model was high and theconsistency was good.(4)The fine environment of hypertensive prevention and management should beconstructed through using all kinds of advantages from universities and colleges--"asmall society". The comprehensive control measures with drugs as leadership shouldbe done well, aiming at those influencing factors of blood pressure control whichwere above-mentioned; so as to promote patients’ individual therapy and population’scomprehensive prevention and control.Special funds and facility investments ofhypertensive prevention and control should be increased. These measures were verynecessary.
