The Research About The Localization And Application Of The Rapid Geriatric Assessment

Objective:To learn the accuracy and effectiveness of the scale named Rapid Geriatric Assessment,we did the translation of the Rapid Geriatric Assessment(RGA)and the examination of reliability and validity of the Chinese version of the scale.And we applied the Chinese version of the Rapid Geriatric Assessment among the community-dwelling aged,in Lanzhou of Gansu province,to learn the situation about the elderly's frailty,sarcopenia,nutrition and cognition,as well as the influenced factors to these syndromes.We look forward that the results of this study may provide some support and foundation for the aged policy or medical services one day.Methods:Cross-sectional observation research method.1.Translation the Rapid Geriatric Assessment and examination its validity and reliability:Followed the“Brislin Translation-back Translation method”,we finally formed the Chinese version of Rapid Geriatric Assessment for test.And the researcher conducted the pretest on the RGA in thirty volunteered aged who were aged over 65 in community.After cultural adjustment and vision,we decided the final Chinese version of Rapid Geriatric Assessment(RGA-C).We valued the reliability of the scale with Cronbach's a coefficient,test-retest reliability and Guttman's half-symmetric coefficient.Also we tested the validity of the scale with the correlation between the subscales,content validity index(CVI)of the content validity and through the confirmatory factor analysis(CFA)and exploratory factor analysis(EFA)to describe the construct validity.2.Application of the Chinese version of Rapid Geriatric Assessment:With convenient sampling,we recruited the community-dwelling aged who were satisfied the selected standards,from Octomber of 2018 to February of 2019,to finish the survey of questionnaire.Descriptive statistics are used to describe the general sociodemographic data.Also we analyzed the factors which may affect the frailty,sarcopenia,nutrition and cognition of the elderly.Results:The first part:The validity and reliability of the Chinese version of Rapid Geriatric Assessment1.Validity:? content validity:the content validity index(CVI)of the all items of the RGA-C was 0.865,while the CVI of four subscales ranged from 0.872 to 0.896.The I-CVI of the scale was 0.800?1.000 and the S-CVI/Ave was 0.988.? construct validity:The ratio of chi-square and degree of freedom of confirmatory factor analysis is 2.582,and there are four factors were extracted through the exploratory factor analysis with the factor loading of each item of the scale were above 0.5.The cumulative variance explained is 58.433%.2.Reliability:? Internal consistency reliability:The Cronbach'a coefficient of each subscale was 0.511?0.809;? Test-Retest reliability:The test-retest reliability of each subscale was 0.802?0.850;? The split-half reliability coefficient:The Guttman's coefficient of each subscale ranged from 0.503 to 0.764,and the Spearman-Brown coefficient of each subscale were 0.525?0.788.The second part:The application of the Chinese version of Rapid Geriatric Assessment1.The status of frailty,sarcopenia,nutrition and cognition among the elderly:? Frailty:The average scores were 1.82±1.37,with 37.2%aged were frail.?Sarcopenia:The average scores were 3.05±2.56,with 45.0%aged were considered sarcopenia.? Nutrition:The average scores were 14.19±2.66,with 51.4%aged were considered in the risk to the weight loss.? Cognition:The average scores were 5.23±2.50,with 52.8%aged were dementia and 24.4%of the aged were mild cognitive impairment.2.The analysis of factors affect the frailty,sarcopenia,nutrition and cognition of the aged:The gender,age,education level and status of disease were concluded as the main reason for the frailty,sarcopenia,anorexia and cognitive impairment of the aged.Conclusion:1.The validity and reliability of the Chinese version of Rapid Geriatric Assessment were acceptable,which can be promoted and applied as a tool for routine screening of the elderly2.More attentions and guidance should be paid to the frailty,sarcopenia,nutrition and cognition of the aged.And it,s also significant to have the timely assessment and scientific direction.3.With the comprehensive geriatric assessment keeps in developing,multidisciplinary exchanges and mutual interaction between scholars are the inexhaustible driving force for promoting its development and perfection.
