| Objective:For CP-FI-CGA(Care partner-frailty index-comprehensive geriatric assessment)questionnaire localization and preliminary application:frist,through screening the geriatrics department patients' frailty and its levels,evaluate the questionnaire's reliability and validity;then,preliminary application of the questionnaire,analysis the patients' frailty degree and the influence factors of the frailty,etc.provide reference for clinical grading nursing care.Methods:1.By looking for foreign literature,selecting questionnaire which compiled by the Canadian professor Goldstein Judah and conformed the Chinese cultural background,the questionnaire is CP-FI-CGA(Care partner-frailty index-comprehensive geriatric assessment),contacting the author through E-mail and getting the authorization of the author.2.Using the modified Brislin translation model to translate the questionnaire,using Delphi expert consultation evaluate the content validity,in the selected four hospital to measure the degree of differentiation,retest reliability,internal consistency reliability,reliability between PingDingZhe,split-half reliability,criterion-related validity and construct validity test of questionnaire.3.Choosing the four general hospitals which set up the geriatric ward of Taiyuan,from June 2016 to August 2016,choosing 300 elderly patients test the CP-FI-CGA questionnaire's reliability and validity;from September 2016 to December 2016,choosing385 patients to gather the data of general information,CP-FI-CGA questionnaire,the Clinical Frailty Scale(CFS)which research and development by Canadian,finally,statistical analysis of data.Results:1.Through Delphi expert consultation the questionnaire's content validity S-CVI/Ave is 0.986.2.The reliability test:retest reliability r = 0.97,reliability between PingDingZhe r =0.995,split-half reliability r = 0.748.3.The validity test:criterion-related validity:simple correlation coefficient of FI and CFS is r = 0.796,correlation coefficient of FI and age is r = 0.486;construct validity the correlation coefficient of all the items score and the FI score is r from 0.019 to 0.636(P <0.05);Internal consistency validity: internal consistency Cronbach's coefficient a = 0.843.4.The evaluation of receptivity:the acceptance of the length of item,the definition of item's content and the content scope is 100%,the acceptance of item's length is 98.67%,the questionnaire's completion time is the(5.45±0.650)min.5.The average frailty-index of 385 patients is(0.32±0.165),the average level of CFS was(5.04±1.483),the correlation coefficient r = 0.819,positively correlated with CFS's level and frailty-index's scores.6.The results of single factor analysis: age,marital status,drug kinds,how much help need,the condition of social support,sleep.these 6 factors score difference was statistically significant;With the increase of age,history of falls and the proportion of more than 5kinds of medication,patients' frailty-index increase;the frailty compare of different sex and different gender(P > 0.05)have no statistically significant.7.Multiple linear regression analysis results: age,drug kinds,how much help need and sleep status were the independent factors which influence FI scores.Conclusion:1.CP-FI-CGA questionnaire comprehensive evaluation patients' each system health condition,comprehensive analysis patients' frailty degree,can be able to reflect the physical and mental state of the patients,can be applied to promote.2.The questionnaire has 43 items,it has well validity and reliability can be used for screening the geriatrics patients with frailty and its levels,meanwhile it can provide reference for clinical grading nursing.3.Age,drug kinds,how much help need,and sleep status were the independent factorswhich influence FI scores,so care-partner and medical staff could be targeted to take steps to slow down the progression of frailty. |
