Research On The Management Model Construction Of Community Cardiac Rehabilitation Education

OBJECTIVE:This study intended to construct the community cardiac rehabilitation education management model based on Chronic Care Model,and explore its feasibility in the community.METHODS:Through literature analysis,this study established a theoretical framework based on Chronic Care Model.According to the connotation of each element of the theoretical framework and the interrelationship between elements,the community cardiac rehabilitation education management model(Model 1st)was initially constructed.The community personnel involved in model 1st was interviewed respectively to discuss the feasibility of the model,and the results of the interview were analyzed.According to the results,each element was revised to form model 2nd.Then,a panel discussion was organized to further discuss the content of each element and their interrelationship.Model 3rd was formed according to the results of panel discussion.The expert group meeting was developed by experts in the field of community health services.Finally,the final version of the community cardiac rehabilitation education management model was established after the expert group meeting.RESULTS:The community cardiac rehabilitation education management model included five elements.The elements,the functions and interrelationships were as follows: service delivery system(including the director of community service station,the dean of community department,and public health doctors and nurses,responsible for their respective duties),clinical information system(establishing electronic files for cardiovascular disease patients,screening high-risk groups,reminding follow-up),self-management support system(setting up self-management log),community(including community residents' committees and community volunteers,carrying out publicity and education activities,mobilizing community cardiovascular diseases patients to participate in),and decision support system(formulating acceptable guidelines for patients according to cardiac rehabilitation guidelines).CONCLUSIONS:Through the organic cooperation and interrelationship between community health service stations,community residents' committees and community volunteers,the community cardiac rehabilitation education management model conducted in the study could suffiently mobilize community resources,and provided the patients with cardiovascular disease with continuous education.
