The incidence rate,mortality rate and disease burden caused by chronic diseases have been more and more serious.According to the data of Yunnan Provincial Center for Disease Control and prevention,in 2017,chronic diseases accounted for 22.9% of the total population of the province,with 328400 reported deaths and 82.5% of deaths due to chronic diseases.The current situation of chronic diseases in Yunnan Province is not optimistic,and the prevention and treatment of chronic diseases has become an important task.Some studies have shown that chronic diseases can be prevented and treated,and the community has unique advantages in the prevention and treatment of chronic diseases,which can be prevented and treated through health promotion and crowd prevention.Therefore,the study of Yunnan Province community chronic disease management is not only conducive to improving the health of residents,but also can provide guidance for community chronic disease management.This paper studies from four aspects: 1.To analyze the current situation of chronic diseases of community residents in Yunnan Province,and further analyze the influencing factors of hypertension,diabetes and stroke.2.Comprehensive assessment of the current situation of community chronic disease management in Yunnan Province,and analysis of the problems in the management of community chronic disease in Yunnan Province.3.Based on the green model,this paper analyzes the community chronic disease management model in Yunnan Province,and analyzes the tendency,contributing factors and strengthening factors that affect the community chronic disease management in Yunnan Province.4.In view of the factors that will affect the management of chronic diseases in Yunnan community,effective countermeasures and suggestions are put forward to improve the management of chronic diseases in Yunnan Community and further improve the health literacy of residents.According to the study,In addition to gender,age and BMI,some socioeconomic variables,such as education level,income and behavior characteristics,such as smoking,drinking,and exercise frequency,will affect the probability of chronic disease in community residents in Yunnan Province.The tendency factors influencing the community management of chronic diseases in Yunnan Province are lack of knowledge and cognition of chronic diseases;the contributing factors are bad health habits,nonstandard health control behaviors,imperfect services and resources related to the prevention and control of chronic diseases in the community;the strengthening factors are the feelings of community health services and social support.Based on the factors that affect the community management of chronic diseases in Yunnan Province,the following suggestions are put forward: strengthen the health education and publicity of the community residents,guide the residents to develop healthy habits;regulate the health control behavior of the patients with chronic diseases,provide more applicable chronic disease management services for the patients with chronic diseases;improve the construction and guarantee of community health resources,and improve the quality of community health services.
