ObjectivesThe purpose of this study was to introduce foreign advanced management of chronic disease,was to explore the effectiveness of Chronic Care Model in the management of metabolic syndrome patients in Chinese community.Methods1.Literature research:Literature review related to chronic disease management was conducted to introduce Chronic Care Model.2.Interventional research:Quasi-Experimental study was used.Three communities of Qiao Si Street,Yuhang District,Hangzhou City,Zhejiang Province were selected.With two communities set as the control group,one community as experimental group.The control group adopted conventional community chronic disease management methods and the experimental group applied Chronic Care Model to manage metabolic syndrome patients respectively.Self-designed general information questionnaire,self-efficacy scale and the MOS 36-Item Short Form Health Survey were used for data collection,and blood samples were collected for biochemical analysis.ResultsAfter 100-day of intervention,the fasting blood glucose level decreased(P<0.05),triglyceride level decreased(P<0.01),and the self-efficacy score increased(P<0.001)in experimental group,the subjects were significantly higher than those in the control group(P<0.05).In terms of quality of life,Physical Function,Bodily Pain,General Health,Vitality,Social Functioning,Mental Health dimensions,the scores in the experimental group were significantly improved.All above indicators were different when compared with the control group.ConclusionsChronic Care Model can partially improve and improve the health outcomes of patients with metabolic syndrome in the community,and more effects need longer interventions to prove.
