Research BackgroundWith the accelerating process of population aging,the change of disease spectrum and medical model,the level of chronic diseases in China is increasing year by year,the cost of medical treatment is high,and the demand for chronic diseases is increasing.The level of urban community health education is far from satisfying the needs of the residents’ diversified chronic health education.Therefore,this study was conducted under the guidance of Guangdong provincial science and technology plan for Guangzhou city community chronic disease health education research.Research ObjectiveThe purpose of this study is to optimize the health education model of chronic diseases in urban communities by using the PRECEDE-PROCEED theory framework,and to analyze the factors affecting the health education of chronic diseases in urban communities from a multi-dimensional perspective.In order to improve the level of community health education workers,to meet the health needs of elderly patients with chronic diseases in community,and to improve the comprehensive prevention and treatment effect of chronic diseases in community.Research MethodIn the theoretical research stage,the literature research method is used to summarize and analyze the literature.In the stage of field investigation,the methods of field research,questionnaire and semi-structured interview are used to understand the development of health education in urban communities.Finally,the influencing factors of health education for chronic diseases in urban communities were analyzed by mathematical statistics.Research Contents(1)This paper summarizes the current situation of health education for chronic diseases in urban communities at home and abroad through literature review.(2)Through the nine links of the PRECEDE-PROCEED model,from two aspects of health education workers and elderly patients with chronic diseases,this paper investigates the knowledge,information,health education needs and satisfaction of the community health education workers and the elderly,and analyzes the factors that affect the tendency,promote and strengthen the health education in the community.(3)In view of the factors of tendency,contributing and strengthening,we put forward scientific and reasonable suggestions so as to improve the level of community health education and meet the health needs of the elderly.Research ResultsThe factors that affect the working level of urban community health education staff are as follows:(1)the tendency factors:the awareness rate of the community health education staff is low and the initiative of autonomous learning is poor;(2)contributing factors:lack of innovation in training mode,lack of educational funds and equipment,and insufficient support from relevant social organizations.(3)strengthening factors:the workload of community health educators is large,the salary system is lack of incentive effect,and the evaluation standard of professional titles needs to be improved.The factors that affect the health education of the elderly patients with chronic diseases in the community are as follows:(1)the tendency factor:the awareness rate of community residents’ health needs to be improved,the attitude and behavior of the residents are "different";(2)contributing factors:the popularization of health education needs to be broadened,the way of health education should vary from person to person,and the way of health education is lack of timeliness;(3)strengthening factors:the elderly health education satisfaction is low,family or friends of health knowledge support less,sedentary entertainment has negative strengthening effect.Research ConclusionThe community should optimize the structure of health education staff and attach importance to health education and training.Increase investment in health education and gradually improve the facilities for health education.The theory of flexible management is introduced to improve the feedback mechanism of health education.Enhance family health and emotional support for the elderly,and improve the physical quality and health literacy of elderly patients with chronic diseases in the community.
