Research objective:Observing the effect of the Communicative Heel Vessel acupuncture on the thickness of tibialis anterior and tibialis posterior of post-stroke patients of strephenopodia by musculoskeletal untrasound and evaluating the effect of the Communicative Heel Vessel acupuncture on lower limb motion by using relevant scales.Research method:64 patients are randomly divided into Communicative Heel Vessel group and control group based on the principle of randomized control.Patients of the control group are given basic treatment of internal medicine disease,conventional acupuncture and rehabilitation exercise.Patients of the Communicative Heel Vessel group are added the Communicative Heel Vessel acupuncture based on the treatment of the control group.All patients are treated for 5 days in a week,except weekend.One treatment cycle includes 4 weeks.The same doctor measures all patients' angle of strephenopodia,Fugl-Meyer Assessment of lower limb,Modified Ashworth Scale of lower limb and Functional Ambulation Classification before accepting treatment,two weeks and four weaks after accepting treatment.The same qualified doctor who works on musculoskeletal untrasound measures the thickness of tibialis anterior and tibialis posterior in quiescent condition of all patients when they haven't accepted treatment and have gotten four weeks treatment respectively.Research Result:(1)Angle of strephenopodia:According to integral analysis,tests of within-subjects effects show that the P value of time is lower than 0.01,P value of time*group is lower than 0.05.Tests of between-subjects effects show that the P value of group is lower than 0.05.Analysed each time point,there is no statistical difference between the two groups before accepting treatment and after getting two weeks of treatment(P>0.05)and there are significant statistical differences between the two groups which have treated for four weeks(P<0.01),it means that the efficacy of the Communicative Heel Vessel group is significantly better than the control group.Analysed in group,there are significant statistical differences in two groups when compared the data of accepting no treatment with getting four weeks of treatment later(P<0.01).(2)Fugl-Meyer Assessment of lower limb:According to integral analysis,tests of within-subjects effects show that the P value of time is statistical difference between the two groups before accepting treatment and after getting two weeks of treatment(P>0.05).The differences have statistical significance between the two groups which have treated for four weeks(P<0.05).Correlation analysis shows that The Modified Ashworth Scale of lower limb of both groups are correlated with changing over time(P<0.05).Gamma coefficients are negative,it means they have negative correlations.The Gamma coefficient of the Communicative Heel Vessel group is lower than the control group,it means The Modified Ashworth Scale of lower limb of the Communicative Heel Vessel group have more correlation with time than the control group.(4)Functional Ambulation Classification:Comparing each time point:there is no statistical difference between the two groups before accepting treatment and after getting two weeks of treatment(P>0.05).The differences have statistical significance between the two groups which have treated for four weeks(P<0.05).Correlation analysis shows that the Functional Ambulation Classification of both groups are correlated with changing over time(P<0.05).Gamma coefficients are positive,it means they have positive correlations.The Gamma coefficient of the Communicative Heel Vessel group is greater than the control group,it means the Functional Ambulation Classification of the Communicative Heel Vessel group have more correlation with time than the control group.(5)Thickness of the tibialis anterior at rest:For the Communicative Heel Vessel group,comparing the affected side of the patient before and after treatment,the difference apparently has statistical significance(P<0.01).For the control group,comparing the affected side of the patient before and after treatment,the difference has statistical significance(P<0.05).For the Communicative Heel Vessel group,comparing the unaffected side of the patient before and after treatment,the difference has no statistical significance(P>0.05).For the control group,comparing the unaffected side of the patient before and after treatment,the difference has statistical significance(P<0.05).Comparing the difference between patients' affected side and unaffected side of both groups before being treated,it shows the difference have statistical significance(P<0.05).Comparing the difference between patients' affected side and unaffected side of both groups after being treated,it shows the difference have no statistical significance(P>0.05).Comparing the difference between the patients' affected side of the Communicative Heel Vessel group and the control group after being treated,it shows the difference have statistical significance(P<0.05).Comparing the difference between the patients' unaffected side of the Communicative Heel Vessel group and the control group after being treated,it shows the difference have no statistical significance(P>0.05).(6)Thickness of the tibialis posterior at rest:For the Communicative Heel Vessel group,comparing the affected side of the patient before and after treatment,the difference apparently has statistical significance(P<0.01).For the control group,comparing the affected side of the patient before and after treatment,the difference has statistical significance(P<0.05).For the Communicative Heel Vessel group,comparing the unaffected side of the patient before and after treatment,the difference has no statistical significance(P>0.05).For the control group,comparing the unaffected side of the patient before and after treatment,the difference has statistical significance(P<0.05).Comparing the difference between patients' affected side and unaffected side of both groups before being treated,it shows the difference have statistical significance(P<0.05).Comparing the difference between patients' affected side and unaffected side of both groups after being treated,it shows the difference have no statistical significance(P>0.05).Comparing the difference between the patients' affected side of the Communicative Heel Vessel group and the control group after being treated,it shows the difference have statistical significance(P<0.05).Comparing the difference between the patients' unaffected side of the Communicative Heel Vessel group and the control group after being treated,it shows the difference have no statistical significance(P>0.05).Conclusion:The Communicative Heel Vessel acupuncture can improve the angle of strephenopodia,the lower limb motor function,the degree of lower limb spasm,the function of gait of post-stroke patients of strephenopodia and it also can improve the morphological of tibialis anterior and tibialis posterior effectively.
