| Objective:Observing the variation of bloodletting therapy on the ankle inversion angle,tibial nerve H-reflex related parameters and scale scores in post-stroke patients of strephenopodia,and exploring the clinical effect of bloodletting therapy on post-stroke of strephenopodia and the neuromuscular system function.Method:The 62 patients who met the inclusion and exclusion criteria were randomly divided into a treatment group(bloodletting therapy)and a control group(conventional acupuncture treatment)with 31 cases in each.Subjects in both groups received basic treatments such as routine medical treatment and rehabilitation.The treatment group was given bloodletting,once every other day,3 times a week;the control group was given conventional acupuncture treatment,daily 1 time,5 times a week,for a total of 4 weeks of treatment.The Clinical Spasticity Index(CSI),Fugl-Meyer Assessment of lower limb(FMA-L),ankle joint varus angle,tibial nerve H-reflex latency and Hmax/Mmax were used to observe the changes of ankle motor dysfunction in post-stroke patients of strephenopodia before and after treatment.The data obtained are statistically analyzed by SPSS22.0 software.Result:(1)Ankle joint varus angle: The ankle joint varus angle after treatment in the two groups was improved compared with that before treatment(P<0.05).After comparison between groups,the angle of ankle joint varus in the treatment group decreased significantly compared with that in the control group(P<0.05).(2)The CSI score: The CSI scores of the two groups after treatment were lower than before treatment(P<0.01).After comparison between groups,the treatment group was better than the control group in reducing the CSI score(P<0.01).(3)The FMA-L score: the FMA-L score of the two groups after treatmentwere improved compared with those before treatment(P<0.01).After comparison between groups,there was no significant difference in the FMA-L score between the two groups(P>0.05).(4)Tibial nerve H-reflex: After treatment,the latency of tibial nerve H-reflex in the two groups was prolonged,and the Hmax/Mmax ratio decreased(P<0.01).After comparison between groups,the treatment group was better than the control group in extending the incubation period and reducing the Hmax/Mmax ratio(P<0.01).(5)Correlation: Before treatment,there is a linear positive correlation between the tibial nerve H reflex Hmax/Mmax ratio and the CSI score(P<0.01),and there is no obvious linear correlation with the FMA-L score(P>0.05);There was no obvious linear correlation between H-reflex latency and the CSI score and the FMA-L score(P>0.05).Conclusion:(1)Both bloodletting therapy and conventional acupuncture can reduce the muscle tension of lower limbs,improve the spasticity of ankle joint and improve the motor function of lower limbs in post-stroke patients of strephenopodia.(2)bloodletting therapy is more effective than conventional acupuncture in improving tibial nerve H-reflex related parameters and reducing the degree of spasticity.(3)The higher the Hmax/Mmax ratio of tibial nerve H-reflex,the higher the CSI score and the higher the degree of lower limb spasm. |
