Objective: Comparing with the use of ordinary mesh + medical glue in the operation,to explore the influence of postoperative pain and seroma for inguinal hernia patients after using self-gripping mesh in operation of TAPP.Methods: A retrospective analysis of 238 cases of laparoscopic inguinal hernia repair(TAPP)between June 2016 and December 2017 in the Department of Gastrointestinal Surgery,Affiliated Hospital of Zunyi Medical College,according to the differences of mesh implantation and differences fixation,cases were divided into self-gripping mesh group(observation group)and ordinary mesh + medical glue(control group),including103 cases in the observation group and 135 cases in the control group.We compare the data of two groups,including general data of age,gender,sputum type,BMI,ASA;and preoperative comorbidities,operative time,intraoperative blood loss,postoperative hospital stay,complications such as seroma,postoperative(3 days,1 month and 6 months)surgical pain(VAS score),recurrence after 6 months of surgery and hospitalization costs.Results: There were no significant differences in age,gender,hernia type,body mass index,preoperative comorbidity,ASA classification,operation time,intraoperative blood loss,and postoperative hospital stay between the two groups(P>0.05),the hospitalization expenses(yuan)of observation group was significantly higher than the control group(12225.2±868.1 VS 9332.7±579.0,P<0.001).In terms of postoperative complications,there were no puncture infection,surgical infection,pulmonary infection,and hematoma in the observation group and the control group.There were 1 case of puncturing in this two groups and there were 6 cases and 7 cases urinary retention in the observation group and the control group respectively.There was no significant difference between puncturing and urinary retention(P>0.05).The incidence of postoperative seroma 6 cases in the observation group was significantly lower than the control group in 22 cases(5.8% VS16.3%,P=0.015).The VAS scores of the patients in the operation area after the 3rd,1st,and 6th postoperative period were 2.2±0.7 VS 2.5±0.9,0.6±0.6 VS 0.8±0.6,and 0.4±0.5 VS0.6±0.8,respectively,the observation group was significantly lower than the control group(P<0.05),and the interaction between the two groups,different time and time and the group were statistically significant(P<0.001),the pain in the observation group was significantly relieved compared with the control group.There were no recurrences in the two groups at 6 months after surgery.Conclusion: Using self-gripping mesh and ordinary mesh in TAPP can both achieve good therapeutic effect in a short period of time for inguinal hernia patients.Compared with the ordinary mesh + medical glue,using self-gripping mesh can reduce the postoperative pain and seroma in inguinal hernia patients after TAPP,but the cost is higher.
