Object:Based on CT 3D reconstruction to make research on the difference of two approachs with acupotomy for the treatment of neck type of cervical spondylosis with prone neutral position and sufficient flexion of the human cervical spine.To provide detailed safety parameters of posterior approach with acupotomy for the treatment of neck type of cervical spondylosis.Methods:Formed images of 50 patients'cervical vertebras?C2C7?with CT 3D reconstruction.Based on the line from the middle of cervical spinous of C2C7,and the perpendicular bisector of cervical spinous.with the prone neutral position and sufficient flexion position,measured the horizontal distances between processus articularis'outer margins,processus articularis'inner margins and the two baselines with bundled software of3D reconstruction system,respectively.Calculated averages and comparative analysis.Results:?1?Compared with the same horizontal intervals from two sides of processus articularis'outer margins to lineae mediana posterior,or to the perpendicular bisector of cervical spinous with the prone neutral position and sufficient flexion position,respectively.There were no statistically significant difference?P>0.05?;In the same case,compared with the same horizontal intervals from two sides of processus articularis'inner margins to them,which has no statistically significant difference?P>0.05?;?2?Between C2C7,compared with the interval of both sides of processus articularis'outer margins to lineae mediana posterior,or to corresponding to the perpendicular bisector of cervical spinous with the prone neutral position and sufficient flexion position,respectively.There were no statistically significant difference?P>0.05?;and in the same condition,compared with the interval of processus articularis'inner margins to posterior midline or to corresponding to the perpendicular bisector cervical spinous.There were no statistically significant difference?P>0.05?;?3?Compared with the horizontal interval of processus articularis'outer margins to lineae mediana posterior,or to corresponding to the perpendicular bisector of cervical spinous?C2C3?C3C4?C6C7?C7T1?with the prone neutral position and sufficient flexion position,respectively.There were no statistically significant difference?P>0.05?;Compared with the horizontal interval of processus articularis'outer margins to corresponding or to the perpendicular bisector of cervical spinous?C4C5?C5C6?,which has statistically significance?P<0.05?.?4?Compared with the same horizontal interval between processus articularis'outer margins and lineae mediana posterior with the prone neutral position and sufficient flexion position,respectively.which has statistically significance?P<0.05?.?5?The secure area is 13.4mm20.7mm from lineae mediana posterior for acupotomy treatment between C2C7 with the prone neutral position.And it is 15.7mm20.9mm with the sufficient flexion position.Conclusion:?1?With the same position and same level,the intervals of processus articularis'outer margins to lineae mediana posterior and inner margins to it are basically symmetrical.And to the perpendicular bisector of cervical spinous are basically symmetrical too.?2?There was no difference between lineae mediana posterior and perpendicular bisector of cervical spinous as baseline when constant the secure area for posterior approach with acupotomy for the treatment of neck type of cervical spondylosis.?3?With the prone neutral position,the intervals are more larger from processus articularis'outer margins to lineae mediana posterior,or to corresponding to the perpendicular bisector of cervical spinous,compared with the prone neutral position.?4?With the sufficient flexion position,the intervals are more larger from processus articularis'inner margins to lineae mediana posterior,or to corresponding to the perpendicularbisectorofcervicalspinous,comparedwiththeproneneutral position.?5?Between C2C7,the secure area is 13.4mm20.7mm from lineae mediana posterior for acupotomy treatment with the prone neutral position.And it is 15.7mm20.9mm with the sufficient flexion position.
