Objective:Schizophrenia is a serious psychiatric disorder in clinic.Studies have shown that abnormal expression of testosterone may affect the neurological function of patients.Therefore,many studies have shown that testosterone is closely related to cognitive function and pathophysiological mechanism in schizophrenic patients.Testosterone expression has become a hot topic in the field of mental disease research.It has been reported that testosterone levels in male schizophrenics are negatively correlated with almost all cognitive functions,especially declarative memory.However,researchers have not found any association between serum testosterone level and cognitive function.In view of this,this study selected male schizophrenic patients,their serum testosterone levels were tested,and analyzed the correation between cognitive function,in order to further clarify the relationship between serum testosterone levels and cognitive function in schizophrenic patients.Methods:From January 2016 to December 2017 in Changzhou Heping Hospital,64 patients with first-episode schizophrenia were selected as the study group,and 57 healthy volunteers were selected as the control group.The cognitive function was assessed and the executive function was determined by the block puzzle;the memory function was tested by the step-by-step continuous addition task;the continuity and concentration of attention were measured by the digital pin test;and the language fluency was measured by the category fluency test.On the next day,fasting elbow venous blood was collected,2 ml in each case,and the supernatant was taken after centrifugation.The expression of testosterone was detected by competitive method using chemical spectrophotometer.Statistical software SPSS17.0 was used to analyze the data.Pearon was used to analyze the correlation between serum testosterone level and cognitive function.Result:(1)The serum testosterone level in the study group was(15.92±4.08)nmol/L,while that in the control group was(15.42 ± 3.12)nmol/L,with no significant difference(P>0.05).In the cognitive function test of the study group,the results of the digital cancellation time and the connection A test were significantly higher than those of the control group.The areas of fluency,spatial breadth,block maps,correct number of continuous step audition and trial number of continuous step audition were significantly lower than those of the control group(all P<0.05).(2)The positive score,negative score,general psychopathology scale and total score of PANSS in the study group were significantly higher than those in the control group,and the difference was statistically significant(P<0.05).(3)There was no significant difference in serum total testosterone level between the study group and the control group(P>0.05).In the study group,the results of time,scope fluency,spatial breadth,connection A,block diagram,correct number of continuous step audition and trial number of continuous step audition were higher than those before treatment,and the difference was statistically significant(P<0.05).The positive score,negative score,general pathological score and total score of PANSS were lower than those before treatment,and the difference was statistically significant(P<0.05).(4)Pearson correlation analysis showed that there was no correlation between testosterone level and cognitive function and PANSS symptom score in healthy males(P>0.05);testosterone level in male schizophrenia patients was positively correlated with digital cancellation time and connection A,negatively correlated with spatial breadth,block maps,continuous stepping auditory correct number,continuous stepping auditory trial number(P<0.05),and category.There was no correlation between fluency(P>0.05);testosterone level was positively correlated with PANSS scale in male schizophrenics(P<0.05).(5)The level of testosterone in male schizophrenic patients was positively correlated with the time of digital write-off and connection A,negatively correlated with spatial breadth,block diagram,correct number of continuous step audition and continuous step auditory attempts(P<0.05),but not with category fluency(P>0.05);the level of testosterone in male schizophrenic patients was positively correlated with PANSS scale(P>0.05).Conclusion:1.There is no difference in serum testosterone level between first-episode male schizophrenia patients and healthy men.2.Male patients with schizophrenia have obvious executive function,memory function,continuity and concentration of attention,and cognitive impairment of language fluency.3.The level of serum testosterone was negatively correlated with spatial memory and attention function in male schizophrenics.3.There is no correlation between testosterone level and cognitive function and clinical symptoms in healthy men.4.There is still correlation between testosterone level and cognitive function and clinical symptoms after treatment,but there is some change.5.In the future,we can try to adjust the testosterone level of schizophrenia patients,so as to improve their cognitive function.6.The level of serum testosterone in the control group may be correlated with its cognitive function.
