Objective: To explore the relationship between the duration of untreated schizophrenia and cognitive function,symptom characteristics of first-episode untreated schizophrenia.To explore the relationship between IQ and symptom characteristics on the prognosis and influence of disease.Methods: Patients with schizophrenia who were hospitalized in Tianjin Anding Hospital from November 2017 to June 2019 were selected.A total of 153 patients with untreated schizophrenia were enrolled in this study,of which 98 subjects were able to complete the baseline information survey and PANSS score after joining the group,and 96 subjects were able to complete the baseline PANSS scale score and Wechsler adult intelligence test.After being treated with a single non-classical antipsychotic for8 weeks,52 subjects were scored with PANSS scale.Group A: a total of 98 schizophrenic patients were divided into two groups according to the length of untreated period.The median baseline questionnaire was 24 months(0-360months).The median was 24 months.The long DUP group was longer than 24 months,and the short DUP group was less than or equal to 24 months.The differences of cognitive function and symptom characteristics between the two groups were analyzed,and whether there was any difference in the degree of symptom improvement after 8weeks of treatment with a single non-classical antipsychotic drug.Group B grouping method: according to the IQ total score of Wechsler Adult Intelligence Test at the baseline stage,the total score was divided into two groups,with 80 points as the boundary,divided into normal IQ group and abnormal IQ group,and compared and analyzed whether there were differences in clinical symptom characteristics.The baseline collection of untreated schizophrenic patients included all the general conditions that may affect the experimental data,taking into account the effects of gender,marital status,age and years of education in the analysis of the experimental data.the differences in the above aspects were compared between the two groups.Results: 1.There was no significant difference in the scores of knowledge scale,similarity scale and mapping scale between the long DUP group and the short DUP group,but there was no significant difference in the score of the wood block scalebetween the two groups(P > 0.05).The scores of speech scale,operation scale and total scale were converted into speech scale,operation scale and total scale,and there was no significant difference between the two groups(P>0.05).There was no significant correlation between DUP and the scores of knowledge scale,similarity scale,mapping scale,block scale,speech scale,operation scale and total scale.Age had no correlation with knowledge scale and mapping scale,but had negative correlation with similarity scale(P <0.05),wood block scale(P < 0.01),speech scale,operation scale and full scale.The years of education had no correlation with the mapping scale,but had positive correlation with knowledge scale,similarity scale and wood scale(P<0.01),and positively correlated with speech scale,operation scale and full scale.2.The difference of psychotic symptoms between the long DUP group and the short DUP group: there was no significant difference in the total scores of PANSS positive scale,negative scale,general scale,PANSS and additional scale between the long DUP group and the short DUP group.Correlation analysis showed that there was no significant correlation between DUP and PANSS subscale scores and total scores.After 8 weeks of treatment,there was significant difference in the total score of PANSS scale and the score of additional scale between the long DUP group and the short DUP group.There was no significant difference in the total scores of positive scale,negative scale and general scale.After 8 weeks of treatment,the PANSS score reduction rate of less than 25% was ineffective,25% of 50% was effective,and more than 50% was effective.The difference of score reduction rate between the two groups was compared,which showed that there was a significant difference between the two groups(P <0.01).3.The relationship between IQ and symptom characteristics: the full scale IQ had no significant correlation with the scores of positive symptom scale and general symptom scale,but had significant negative correlation with the total score of PANSS(P<0.05)and negative symptom scale(P<0.01).There was no significant correlation between verbal IQ and the scores of positive symptom scale and general symptom scale,but there was a significant negative correlation with the score of negative symptom scale and the total score of PANSS(P<0.05).There was no significantcorrelation between IQ and the scores of positive symptom scale and General symptom scale,but there was a significant negative correlation with the score of negative symptom scale and the total score of PANSS.The results of comparison between the two groups: there were significant differences in the scores of negative symptom scale,general symptom scale and total score between the normal IQ group and the abnormal group(P<0.01),but there was no significant difference in the score of positive symptom scale.Conclusion: 1.There was no difference in the span of knowledge,distant memory,abstract ability and general situation ability,visual recognition ability,spatial understanding ability,visual analysis comprehensive ability,spatial construction ability and so on.The length of untreated schizophrenia did not significantly affect the neurocognitive impairment of patients,and there was no significant correlation between them.2.The length of untreated period was not significantly related to the total score of PANSS positive scale,negative scale and general scale before treatment,that is,the length of untreated period had no significant effect on the symptom characteristics of the disease.After 8 weeks of treatment with single non-classical antipsychotic drugs,the total score of PANSS was different,indicating that the untreated period was a factor affecting the prognosis of the disease.3.the scores of verbal IQ,operational IQ and total IQ were significantly correlated with negative symptoms and PANSS total scores,but not with positive symptoms.The lower the IQ of schizophrenic patients,the higher the score of negative symptom scale and the total score of PANSS,the more serious the symptoms,and the more prominent negative symptoms.
