Objective:This paper is preliminarily explore the distribution law of TCM syndrome elements on lumber disc herniation and the relationship with radiology index.It provides a basis for the objectification and standardization of TCM syndrome differentiation for this disease,and attempts to provide a useful reference for TCM clinical syndrome differentiation.Methods:From March 2018 to January 2019,this study collects 172 patients who had lumbar disc herniation from Quanzhou Traditional Chinese Medicine Hospital with detailed record of clinical four-diagnosis information and basic information such as age,gender,and duration of illness.According to “Syndrome Differentiation Scale” which from Zhu Wenfeng’s book 《The Dialectics of Syndrome》,this study extracts the relevant position and nature of this disease,establishes a database and preliminarily explores the distribution of syndromes in lumbar disc herniation by the employ of statistical analysis.Results:1.A total of 172 patients with lumbar disc herniation are embodied in this study which including 102 males(59.3%),70 females(40.7%),with a maximum age of 68 years and a minimum age of 22 years.52 patients(30.2%)with a disease duration are less than3 months;49 patients(28.5%)experience 3 months to 1 year;33 patients(19.2%)endure 1-3years disease course;38patients(22.1%)endure more than 3 years.There are 62 cases have herniated lumbar nucleus pulposus with diameter <3mm,accounting for 36.1%;84 cases with 3mm-5mm diameter,accounted for 48.8%,and diameter of herniated lumbar nucleus pulposus >5mm are 26 cases,accounting for 15.1%.2.The syndromes distribution on lumbar disc herniation of this study: according to the statistics,the kidney 95 cases(33.9%);the liver 64 cases(23.0%);the spleen 57 cases(20.4%);the meridian 32 cases(11.5%);the muscle and bone 13 cases(19.6%);the stomach 11 cases(3.9%);the gallbladder 4 cases(1.4%);the skin 2 cases(0.7%);the heart 1 case(0.3%);the lung 1 case(0.3%).Among them,there are 77 cases(44.8%)with single disease syndrome,89 cases(51.7%)with double disease syndromes,and 6 cases(3.5%)with three or more disease syndromes.The frequency of extracted disease syndromes is statistically analyzed.Blood stasis are 78 cases(21.3%),wet 51 cases(13.9%),yang deficiency 42 cases(11.4%),qi stagnation 40 cases(10.9%),heat 37 case(10.1%),cold 34 cases(9.4%),qi deficiency 31 cases(8.4%),yin deficiency 30 cases(8.2%),wind11 cases(3.1%),arthralgia 7 cases(1.9%),blood deficiency 3 cases(0.8%),sputum 1 case(0.3%),yang hyperactivity 1 case(0.3%).Among them,77 cases(44.8%)with double disease nature,53 cases(30.8%)with single disease nature,34 cases(19.8%)with three disease nature,and 8 cases(4.6%)with four or more disease nature.3.The syndromes combination and distribution on lumbar disc herniation of this study: The four syndromes accounts for the largest proportion,61 cases in total,accounting for 35.5%,followed by 46 cases of three syndromes,accounting for 26.7%,six syndromes and above are rare,only 9 cases,accounting for 5.2%.The double syndrome and the five syndromes account for 18.6% and 14.0% respectively.There is a significant difference in the combination of age and age(P<0.05),but no significant correlation with the course of disease and herniated lumbar nucleus pulposus(P>0.05).The different characteristics of the syndrome combination were analyzed.The highest frequency of deficiency and excess mixed is 78 cases,accounting for 45.3%,followed by 70 cases of empirical patients,40.7%,and fewer cases of deficiency syndrome,24 cases,accounting for 14.0%.In terms of the nature of the combination of syndromes:males are mostly empirical,and women are mostly mixed with deficiency and excess.As the age growing and longer course of disease,the empirical patients gradually turn to deficiency and excess combined syndrome and mainly appear deficiency syndrome.Most patients with deficiency syndrome have herniated nucleus pulposus with small diameter,while patients have herniated nucleus pulposus with large diameter mainly appear excess syndrome.The nature of different syndrome combinations had a certain correlation in gender,age,duration of disease,and herniation of lumbar nucleus pulposus(P<0.05).Conclusion:1.Kidney,liver,spleen and meridian are the main disease locations of lumbar disc herniation,and the patients with double disease locations are the majority.Besides the disease combination of kidney with liver,spleen and meridian is the most common,which indicates that the disease has a close relationship with kidney.Blood stasis,yang deficiency,dampness,yin deficiency,qi stagnation,cold,heat and qi deficiency are the main disease syndromes of lumbar disc herniation,among which the syndrome of blood stasis is significantly higher than other disease syndromes.Patients with double disease locations are mainly combining with blood stasis,qi stagnation,qi deficiency and other diseases,which suggesting that blood stasis is the key factor in the development of this disease and cause the disease combined with other syndromes.2.The combination of syndromes of lumbar disc herniation is diversified,among which four syndromes and three syndromes are mostly involved,besides involves doub le syndrome,five syndromes and six syndromes or more.There was no significant cor relation between the combination of syndrome and the course of disease and the degre e of herniated lumbar nucleus pulposus.As the age increased,the number of syndrom e combinations increased.3.Patients with lumbar disc herniation are mostly mixed with deficiency and excess,followed by empirical patients,and patients with deficiency syndrome are less common.It suggests that the pathological features of this disease are virtual standards.The nature of different syndrome combinations had a certain correlation in gender,age,duration of disease,and herniation of lumbar nucleus pulposus.
