Background The condition of preeclampsia was complicated and changed rapidly.The physiological changes of delivery and postpartum as well as all kinds of pessimal stimulation can lead to the aggravation of the disease.It is very important to monitor and evaluate the condition of prenatal,intrapartum and postpartum to clear the severity and progress of the diseae and to intervent timely to avoid the occurrence of adverse prenancy outcomes.The application of clinical epidemiology through a retrospective analysis can provide excellent evidence for the prediction,diagnosis and prevention of the adverse outcomes of pregnant women with preeclampsia and perinatal infants.Objective In this paper,we retrospectively analysed 255 cases with severe preeclampsia in our hospital to explore the related risk factors for adverse pregnancy outcomes.Methods We choosed 255 cases of severe preeclampsia patients who deliveryed in our hospital from January 2012 to April 2016 as the research objects,and divided all cases into A group with good outcomes and B group with adverse outcomes.Results1.Group B patients were more likely to have higher diastolic pressure and earlier gestational age at delivery and onset gestational weeks.The difference was statistically significant(P < 0.05).Other general information was of no significant difference(P >0.05).2.Compared with group A,group B patients were more likely to have higher levels of hemoglobin(HGB),D-dimer,aspartic acid amino transferase(ALT),alanine amino transferase(AST),serum creatinine(SCR),urea(UREA)and lactate dehydrogenase(LDH)content,and to have lower levels of the platelet count(PLT),fibrinogen level(FIB)and albumin(ALB)content.The difference was statistically significant(P <0.05).Prothrombin time(PT)was shorter than that of A group,The difference was statistically significant(P < 0.05).3.Collected all risk factors into logistic regression and the result showed that gestational age at delivery,onset gestational weeks,HGB,FIB,PLT,ALT,AST,SCR and UREA were the independent risk factors of adverse pregnancy outcomes.The earlier gestational age at delivery,onset gestational weeks,the higher HGB,FIB,PLT,ALT,AST,SCR and UREA,the lower PLT and FIB,the greater the likelihood of adverse pregnancy outcomes.Conclusion Gestational age at delivery,onset gestational weeks,HGB,PLT,FIB,ALT,AST,SCR and UREA were the independent risk factors for the occurrence of adverse pregnancy outcome in severe preeclampsia.The earlier gestational age at delivery,onset gestational weeks,the higher HGB,FIB,PLT,ALT,AST,SCR and UREA,the lower PLT and FIB,the greater the likelihood of adverse pregnancy outcomes.In clinical work,we should pay attention to the abnormal changes of these indicators and take effective intervention measures to prevent the development of the disease and improve maternal and fetal outcomes.
