Objective: To investigate the clinical characteristics of patients with Sweet's syndrome(SS).To analyze the differences in the pathogenesis,clinical manifestations and prognosis of SS patients with or without systemic involvement.To summarize the clinical experience for rapid assessment of patients' condition and timely and effective individualized treatment.Methods:Clinical data of 54 patients with Sweet's syndrome were retrospectively analyzed,including general information,clinical manifestations,laboratory examination,histopathological examinations and treatment.The patients were divided into two groups according to whether the systemic involvement was associated or not.SPSS 25.0 statistical analysis software was used to analyze the data.Results: 1.General information: In this study,22 patients were involved in systemic involvement,and 32 patients had only skin lesions.The ratio of male to female in 54 patients with Sweet's syndrome was 1:1.7.There was no significant difference in gender between the two groups(p>0.05).The mean age of onset of the patients was 53.22±12.74 years old;the mean age of onset of the males was 53.10±13.07 years old;the mean age of onset of the females was 53.29±12.94 years old.There was no significant difference in the age of onset between men and women(p>0.05).There was no significant difference in gender between the two groups of whether it is associated with systemic involvement or not(p>0.05).The incidence of systemic involvement in SS patients with malignant tumors is 80%.2.Clinical features: 9 cases(16.67%)rash only involved one site,45 cases(83.33%)involving more than 2 sites,skin lesions involving multiple sites with systemic involvement(p <0.05).Seven patients had oral ulcers.The incidence of oral ulcers with or without systemic involvement was 27.27% and 3.12%,respectively,and the difference was statistically significant(p<0.05).There were 21 cases(38.89%)with fever,9 of them were involved in system involvement,and there was no significant difference in the incidence of fever with or without systemic involvement(p>0.05).There were 4 cases(7.41%)with eyes involvement,11 cases(20.37%)with joint pain,and 4 cases(7.41%)with muscle pain.Atotal of 54 patients received blood routine results: 20 cases of elevated white blood cells(37.04%),26 cases of neutrophil increased(48.15%);45 cases of C-reactive protein(CRP)were collected: 37 Example(82.22%);49 cases of erythrocyte sedimentation rate(ESR)were collected: 39 cases(79.59%)were elevated.There was no significant difference in statistical analysis between the accompanying or associated systemic involvement(p>0.05).Histopathology showed that dermal papillary edema of different degrees was observed in the dermis,and inflammatory cell infiltration of different degrees around the superficial blood vessels of the dermis,mainly neutrophil infiltration.3.Treatment and prognosis: glucocorticoid is the first-line treatment drug,colchicine,total glucosides of paeony capsules,antibiotics and other drugs can also achieve good efficacy.There were 20 cases of relapse(37.03%),and the difference of recurrence rate between the two sexes was not statistically significant(p>0.05).There was no significant difference in recurrence rate and length of hospitalization between patients with or without systemic involvement(p>0.05).Conclusion:1.Oral mucosal damage may indicate the occurrence of systemic involvement;2.When the skin lesions involve multiple sites,it is more likely to be associated with systemic involvement;3.There was no significant difference in length of stay and recurrence rate among SS patients with or without systemic involvement.
