Objective:To investigate the pulmonary involvement in systemic sclerosis (SSc) and to conclude the clinical features of SSc with pulmonary involvement, which may be helpful for diagnosis of SSc with lung involvement.Methods:using the designed questionnaire, and retrospectively analyzing the clinical features of77cases of SSc, including sex, age, disease duration, scleroderma subsets, the first symptom, clinical manifestations and laboratory examinations, etc.Result:1、Among the77patients enrolled in our study, the disease had a4.1:1female predilection with the onset age of (43.12±14.23) years and typically occurred in the third to fifth decades of life.39patients (50.6%) suffering from SSc developed pulmonary involvement, including36cases of interstitial lung disease (ILD)(92.3%). There was no significant difference between SSc patients with and without pulmonary involvement in sex, age, disease duration,scleroderma subsets;2、Fiber trabs shadow and reticular patters were the most frequent radiological manifestations in SSc patients with pulmonary involvement;3、Cutaneous lesions is the most frequent first symptom in SSc patients without pulmonary involvement(P=0.005), including pruritus, skin flushing and skin sclerosis. There was no significant difference between SSc patients with and without pulmonary involvement in Raynaud’s phenomenon, musculoskeletal lesions, and lung lesions;4、SSc patients with pulmonary involvement took higher risk of skin depigmentation, chronic cough, shortness of breath (P=0.004, P=0.002, P=0.000), Antitopoisomerase I antibody(anti-Scl70)(P=0.01) and elevated levels of IgA (P=0.03),while there was no the significant difference between the two groups in Rodnan score, vascular involvement,musculoskeletal involvement, cardiac, renal and liver involvement and other laboratory examinations.Conclusion:1、SSc had a4.1:1female predilection and typically occurred in the third to fifth decades of life; 2、Pulmonary involvement occurred in about50.6%of SSc patients and its most frequent radiological manifestations were the reticular patters of the double lower lung lobes;3、The SSc patients whose first symptom was Cutaneous lesions were less prone to pulmonary involvement. SSc patients with pulmonary involvement took higher risk of skin depigmentation, chronic cough and shortness of breath;4、SSc patients with pulmonary involvement took higher risk of anti-Scl70, elevated levels of IgA compared with those patients without pulmoary involvement.
