Purpose:Retrospective analysis:Analysis of epidemiological,clinical features,recurrence,and laboratory examinations of 53 patients with Sweet Syndrome.The related indicators of patients with recurrence and non-recurrence were compared to further explore the related factors of recurrence of the disease.Methods:1.Participants:This study collected data on the gender,age,clinical manifestations,histopathology,and laboratory examinations of 53 patients with Sweet syndrome who were hospitalized in the dermatology department of The First Affiliated Hospital of Dalian Medical University from December 1997 to October 2018.At the same time, the 53 patients were followed up by case record and telephone follow-up.18 patients were lost to follow-up,35 patients were obtained,and 35 patients were divided into recurrent group and non-recurrent group.A controlled study was conducted to analyze the recurrence factors.2.Statistical analysis:Normal analysis of two consecutive variables was performed using the Kolmogorove-Smirnov test and age using an independent sample t test. White blood cell count,neutrophil percentage,and neutrophil count using independent sample t-test and ROC curve.The sum of the patients in the two groups was tested by rank sum test,and the clinical manifestations were computed using Fisher's exact probability method.P less than 0.05 was statistically significant.Results:1.The average age of onset of 53 patients was 60.09±14.06 years old,21?39.6%?for male patients and 32?60.4%?for female patients;6 patients had upper respiratory tract infection before onset,1 patient had urinary tract infection before onset,and 1 patient had synovitis before onset,2 patients were diagnosed as malignant tumors before or after onset;There were 17 patients?32.1%?with body temperature>38°C;5 patients?9.4%?with a single skin lesion and 48 patients?90.6%?with multiple skin lesions.There were only 4 cases?7.5%?involving the lesions of head and neck,only 2 cases?3.8%?involving the trunk,17 cases?32.1%?involving only the extremities, and 30 cases?56.6%?involving two or more places;16 cases?30.2%?of pseudo-blister,4 cases?7.5%?of pustular lesions,and 8 cases?15.1%?with increased skin temperature;For laboratory indicators of 53 patients,there were 27?50.9%?white blood cell counts>8×109/L,and 24?45.3%?with neutrophil percentage>70.There are 40 cases?75.5%?with pathology directly in line with the disease. Pathology was not directly diagnosed,and 13 cases?24.5%?required clinical diagnosis.After treatment with glucocorticoid and immunosuppressants,the rash can fade away.Among the 53 patients,35 patients completed follow-up,and 17 patients ?48.6%?relapsed by the end of 2018.2.Contrast results in patients with recurrent and non-recurrent groups:The mean ages of the non-recurrent group and the recurrent group were 61.11±13.82 years old and 61.82±13.14 years old,respectively.There was no statistical difference?P=0.877?. There were 10 males and 8 females in the non-recurrent group,3 males and 14 females in the recurrent group,and the difference was statistically significant ?P=0.023?.There was no obvious cause of the disease in both groups.In most cases the sites of the lesions are multiple;The white blood cell count,neutrophil percentage and neutrophil count in the non-recurrent group were higher than those in the recurrent group,and WBC and N were statistically different.In the ROC curve, AUGWBC=0.719,AUGNEUT%=0.657,AUGN=0.735.Conclusions:?1?Sweet syndrome is a rare idiopathic disease,and the incidence of female patients is higher than that of males;?2?83.0%of patients aged 40 to 80 years old,mainly in middle-aged and elderly patients;?3?The erythrocyte sedimentation rate increased in 83.3%of patients,and the C-reactive protein increased in 70.8%of patients,which showed sensitivity;?4?17 patients?48.5%?relapsed in the group,indicating a higher recurrence rate of the disease;?5?Female patients are more likely to relapse than males;?6?Teukocytosis and neutrophilia have diagnostic value for Sweet syndrome,and the white blood cell count and neutrophil count had moderate diagnostic accuracy for Sweet syndrome.
