Objctive: A retrospective systematic analysis of 14 patients with L-shaped incision in our department was made,and the application of L-shaped incision in thoracic surgery was summarized.Methods: From 2011 to 2018,14 patients underwent "L" incision surgery in thoracic surgery department of our hospital were retrospectively analyzed.Among them,13 patients underwent "L" incision through manubrium sterni and 1 patient underwent high posterolateral "L" incision.The basic case information,pathological types and other data of the patients were collected.The data were analyzed by Excel,and the operation methods were summarized through data analysis.Results: 1.14 cases have successfully completed the operation,11 cases have been completely removed the lesion site,2 cases have been palliative resection,and 1 case has been simply explored.All cases have no perioperative death.2.Among the 13 patients undergoing anterior approach,1 patient underwent total sternotomy due to tumor invasion of the middle and upper mediastinum,and the incision was "T".Two patients suffered from thyroid cancer invading the upper mediastinum.The tumor could not be completely resected through a simple neck incision.The neck incision was curved,and the remaining incisions were the same as the "L" incision through manubrium sterni.3.The operation time(140 ~ 470 min),intraoperative Fill volume(2100~5150ml),postoperative hospital stay(7 ~ 22 days),intraoperative blood loss(30 ~ 800 ml)and other data may be larger than conventional operation methods.4.14 patients suffered from Honor syndrome,upper limb dysfunction,pleural effusion and other complications after operation,which were all improved after active treatment.Conclusion: The "L" incision can be applied to upper mediastinum tumor,tumors in the cervical-thoracic junction area and tumors whose main body is located at the thoracic entrance,or tumors invading the cervical-thoracic junction area,with good effect.The L-shaped incision is more suitable for the thoracic anatomical structure and can fully expose the operation area,thus providing feasibility for the complete resection of tumors and pathological parts.
