| ObjectiveTo explore the clinical value of Video-Assisted Thoracoscopic Surgery (VATS) in the diagnosis and treatment of mediastinal tumor.MethodsCollect the Clinical data of 116 patients underwent Video - Assisted Thoracic Surgery (VATS)for mediastinal diseases from Jul.2004 to Dec.2008 in Fujian Provincical Hospital, compare to other 48 cases received conventional thoracic surgery in the same period. Then explore the clinical value of VATS in the diagnosis and treatment of mediastinal dise-ases.Results81 cases completed the tumor resection totally by VATS,19 cases received Video-assisted mini-thoracotomy, 11 patients only received tumor biopsy by VATS, and other 5 patients by conventional thoracic surgery. all the diagnosis have been confirmed by pathology. The average operation time, blood losses, chest drainage tube indwelling time, volume of chest tube drainage, mean frequency of analgesic treatment and length of staying in hospital after surgery in the VATS group were less than those in the conventional thoracic surgery group. The VATS group respectively were 2.02±0.77hours,56.5±32.3ml,1.5±1.0days,136.8±161.4ml,1.41士0.84 times,8.2±4.3days, while the conventional thoracic surgery group respectively were2.86土0.49hours,227.9士113.0ml,5.63土2.57days,253.37土121.59ml,2.82士1.17 times,14.17土1.75days, and the differences were statistically significant (P <0.001). In the two groups ,the incidence rates of post-operative complications were respectively 4.3% and 14.6%, (P<0.05 ).During the following-up there was 1 patient who had chronic pain in the VATS group,and 7 patients in the conventional thoracic surgery group,among which 5 patients had upper limb movement disorder. This difference was statistically significant (P <0.005)too.ConclusionVATS can not only complete tumor excision, and when the tumor can not be resected, the biopsy can be carried out too. Meanwhile, compare to the conventional thoracic surgery, VATS has many obvious advantages ,such as surgical safely, small incision, little surgical injury, less bleeding, shorter surgical time, less indwelling time of chest drainage tube, less drainage volume, less pain and decreased rates of chronic complications, little impact on upper limb exercise,all these improve the quality of life of patients, and do great help to the rehabilitation of patients, so they stay in hospital for shorter time. In a word, VATS have great clinical value in diagnosis and treatment of mediastinal tumor, It is well worth developing and promoting. |
