The Effect Of Measurement Parameters Of Tumor And SCC-Ag Changes On The Survival And Prognosis In Middle And Advanced Cervical Cancer

Abstract/Summary:
ObjectiveThe purpose of this study was to investigate the effect of specific measurement parameters such as tumor diameter,tumor volume,tumor volume reduction rate(TVRR)and the squamous cell carcinoma antigen(SCC-Ag)changes on the survival and prognosis of patients with middle and advanced cervical cancer undergoing concurrent radiotherapy and chemotherapy.MethodsThis retrospective study collected 203 cases of IIA-IVA stage cervical squamous cell caicinoma who were first diagnosed in our hospital from January 2011 to March 2015.All patients’clinical data and imaging information before and after treatment had been collected and each parameter value calculated by using 3DSlicer software.The pre/post-treatment tumor diameter(TDpre/post),tumor volume(TVpre/post),SCC-Ag(SCCpre/post),and TVRR,SCC-Ag reduction rate(SCCRR)were analyzed and evaluated on prognosis of cervical cancer.ResultsThe median follow-up among all 203 patients was 69 months.The 5-year overall survival rate(OS)and disease progression-free survival rate(PFS)were 69.5%and 64.5%,respectively.Univariate analysis showed that the TDpre/post,TVpre/post,TVRR,SCCpre/postand SCCRR were all related to OS and PFS(P<0.05).Multivariate Cox regression analysis showed that the TDpre(HR=0.373,P=0.028),TDpost(HR=0.376,P=0.003)and SCCpost(HR=0.374,P=0.001)were independent prognostic factors of OS.The TVRR(HR=2.998,P<0.001),SCCpre(HR=0.563,P=0.041)and SCCpost(HR=0.253,P<0.001)were independent prognostic factors of PFS.In addition,there was a significant positive correlation between measurement parameters of tumor and SCC-Ag(P<0.05).ConclusionThe TDpre/post,TVpre/post,TVRR,SCCpre/post,and SCCRR were prognostic factors in middle and advanced cervical cancer.TDpre/post and SCCpost were more important factors which affect the prognosis.The TVRR and SCCpre/post were closely related to the disease progression.In addition,further studies about the correlation between measurement parameters of tumor and SCC-Ag should be conducted in the future.
