Objectives:1. To investigate the communication status of helicobacter pylori in different groups of a industrial area in Gansu province;To find out the character of Hp distribution and provide epidemiological data for the diseases such as chronic gastritis.2. Throughout the detection and analysis of saliva in the patients of different gastrointestinal diseases, we will built up the correlation of the oral helicobacter pylori with the different gastrointestinal diseases and provide theoretical basis for clinical prevention and treatment of gastrointestinal diseases.3. Expect to establish a noninvasive diagnostic method of Helicobacter pylori, the clinical diagnosis of gastrointestinal diseases provides an effective monitoring method translation4. Studies in patients with gastrointestinal disease, different periodontal status of Hp in the oral cavity on the impact of colonization, and discuss the relationship between periodontal diseases and Gastrointestinal diseasesMethods:1. In August 2009 to April 2010 period, An investigation on the gastric Hp positive rate of 1465 people, who participated in a physical examination, by the method of [14C]-Urea Breath Test,grouped patients according to the differences of work, gender, at last, the Hp-positive rates were compared. And statistics using SPSS 16.2. In November 2009 to April 2010 period,173 patients who complained of gastrointestinal symptoms from Department of Gastroenterology of Baiyin First People's Hospital were elected and received gastroscopy. Meantime, made the oral examination, collected saliva, and made detailed record of oral and gastroscopy results. At the same time,128 patients from Department of Stomatology of baiyin First People's Hospital were established as the control group, who had no gastrointestinal symptoms and received the oral examination. Then the oral examination results were recorded.173 patients with gastrointestinal disease were grouped according to age, gender, periodontal status and types of gastrointestinal disease. Hp were detected from saliva samples of all patients by culturation in vitro. The Hp-positive rates in different groups were statistically analysed. Results:A total of 1465 people who participated the physical examination was composed of 1007 males and 458 females in the age range 25-55. The Hp-positive rate in stomach was 55.36%, The Hp-positive rate in stomach in public servant group was 54.55%, the Hp-positive rate in stomach in non-public servant group was 55.99%, there was no significant difference. The Hp-positive rate between the male (56.60%) and female (52.62%) was not significant difference. But the Hp's severe infection rate in females(17.25%) is significantly higher than males (5.76%), Of the patients with gastrointestinal disease female account for 57.80%, male account for 42.20%, there is a remarkably difference between them(P<0.05).The Hp-positive rate in all 173 patients was 40.46% and the difference between male and female showed significant (P<0.05); The detectable rate of Hp was 56.72% in the age range 45-64, which is significantly higher than two younger age groups (P<0.05); The Hp-positive rate in patients with atrophic gastritis was 77.78%, of which the difference was significantly higher than superficial gastritis group and gastric & duodenal ulcer group respectively (P<0.05); The Hp-positive rate in healthy periodontia group was 15.38%, while that in periodontitis group was 72.73%(P<0.05).Conclusion:The Hp infection rate of inhabitant in the city of baiyin is upper accounted for 55.36% that close to the national average. The Hp infection does not related with sex. But in Hp's severe infection of the female generally higher than male. And the number of female patients with gastrointestinal disease are also far higher than male. So we presume that Hp is easier to grow or reproduce in the women's stomach, resulting in gastrointestinal diseases.the difference of Hp-positive rates in saliva between the gastrointestinal disease group and the control group is indistinctive(P>0.05). So Hp is a conditional pathogen, it exists widespread in the oral of health patients and gastrointestinal disease patients, and it would not cause disease under normal circumstances.The detectable rate of Hp in the atrophic gastritis group, which is significantly higher than other groups, The atrophic gastritis patients accompanied atypical hyperplasia of gastric mucosa or intestinal metaplasia have higher the rate of Hp-positive in saliva. the detectable rate of Hp from saliva is closely related to the types of gastrointestinal disease in patients.there is significant difference between the age and sex group. Self-awareness of oral health for men is weaker than women, and the number of smokers more likely than women. gingivitis, dental calculus detection rate is relatively high in men; and the patients in the age range 45-64 have poor oral hygiene and different levels of periodontal diseases. Therefore, oral health status can be directly or indirectly impact the a bodily system especially digestive system, and resulte in a higher incidence of gastritis, gastric ulcer, gastric cancer and other gastrointestinal diseases.the detectable rate of Hp from saliva is closely related to the types of gastrointestinal disease in patients, and it is correlated with the periodontal diseases as well; the oral cavity with periodontal diseases is an ecological niche of Hp which might be an important cause for occurrence and re-occurence of gastrointestinal diseases.
