Objective: In order to understand the status quo of anxiety and depression among inpatients in different clinical departments in general hospitals,the positive rate of anxiety and depression among inpatients in different clinical departments was compared,and the causes and main influencing factors of anxiety and depression among inpatients were analyzed.Methods:According to the standard set,adopt the method of random sampling selection in January 2018 to October in a general hospital in Qingdao different clinical departments(respectively,general surgery,respiratory,orthopedics,neurology,digestive internal medicine,oncology,heart,liver and gallbladder surgery,urology,with pain)in hospitalized patients 582 cases,using self-made general situation questionnaire,cases of type with self-reported depression scale(SDS),type of self-evaluation of anxiety scale(SAS),Hamilton anxiety scale(HAMA)and Hamilton depression scale(HAMD)to assess,The diagnosis was made according to the international Classification of diseases(ICD-10)diagnostic criteria.Results:In the clinical departments investigated,the total score of Hamilton anxiety scale(HAMA)was more than 14,the standard score of SAS was more than 50 and met the diagnostic criteria of ICD-10.The total positive rate of anxiety disorder was 37.46%,the total positive rate of depression disorder was 34.02%,and the comorbidity rate of anxiety and depression was 24.23%.The prevalence of anxiety was the highest(59.09%)in the department of pain,and the lowest(18.60%)in the department of hepatobiliary surgery.The prevalence of depression was the highest(62.69%)in the department of oncology,and the lowest(16.67%)in the department of respiratory medicine.In terms of comorbidity,the department of oncology(43.28%)had the highest comorbidity rate,followed by the department of pain(43.18%).There was no significant difference between the diagnostic results of the zong depression/anxiety scale and Hamilton depression/anxiety scale(P >0.05),and the two diagnostic methods had a high degree of coincidence.In the general investigation and analysis,the positive rate of the patients with female,low educational level,divorced or widowed marital status,poor family relationship,poor economic status,positive family history of anxiety and depression and recent major life events was higher(P <0.05).There was no difference in the positive rate between different age groups(P > 0.05).Conclusion:Among the inpatients in general hospitals,the prevalence rate of anxiety and depression is higher,and there are certain differences between different clinical departments.The prevalence rate of anxiety is higher in patients with acute disease courses such as surgery,while the prevalence rate of depression is higher in patients with chronic diseases(such as oncology department).However,the recognition rate and treatment rate of anxiety and depression by non-psychiatric specialists in general hospitals are both low,and the prognosis of patients with anxiety and depression is often worse than that of patients without anxiety and depression,and the average hospital stay and average hospitalization cost are also higher than those of general patients.Such as the pain caused by anxiety depression also accounts for a considerable proportion of body discomfort,such as the spirit of the specialist can such symptom and somatic disease symptoms caused by identifying,often leads to the medicine does not suit the,check "excessive",and so on and so forth,waste the existing medical resources,also exacerbate the doctor-patient contradiction,influence the patient's medical treatment experience.At the same time,patients' anxiety and depression are affected by a variety of factors,and there are significant differences in the degree of influence of different factors.Therefore in general hospital should strengthen their with anxiety depression in patients with psychological problems such as attention,understand the risk factors,and establish the necessary treatment and contact of the consultation system,to improve the recognition rate of anxiety,depression,mental specialists in hospitalized patients with anxiety depression timely intervention,to improve the prognosis of patients in hospital,to avoid the occurrence of adverse events.Meaning: At present,the investigation of anxiety and depression status of patients in domestic general hospitals is mostly limited to the outpatient department,and most of them are studies on anxiety and depression in a certain specialty,and there is still a lack of large sample and multi-center data on the status of anxiety and depression in inpatients.The results of this study are conducive to understanding the current psychological status of inpatients in general hospitals,mastering the main influencing factors,supplementing the current data of mental health in China,and achieving early detection and early intervention for inpatients with certain risks.To promote the construction of new medical model,lay a good foundation for the new medical model of "biology-psychology-society"(ten concepts and three levels)in China.
