Objective The purpose is to explore the anxiety and depression status and influencing factors of diabetic patients with or without retinopathy,which provides certain theoretical basis to wonder if carrying on the corresponding psychological intervention for the diabetic patients with or without retinopathy.Methods A total of 225 diabetic outpatients or inpatients admitted to the departments of Ophthalmology and Endocrinology of Chaohu Hospital of Anhui Medical University from January 2018 to July 2019 were enrolled in the study,including 97 patients with retinopathy(diabetic retinopathy group,DR group)and 128 patients without retinopathy(non-diabetic retinopathy group,NDR group).General information of the patients was collected through questionnaire survey,including gender,age,course of disease,average monthly income,education level,occupation and marital status,as well as the course of diabetic retinopathy,examination methods and treatment methods.Hamilton Anxiety Scale(HAMA)and Hamilton Depression Scale(HAMD)were used to assess the incidence and scores of anxiety and depression,respectively.The incidence and scores of anxiety and depression were compared between the two groups.Univariate analysis and multiple linear regression were used to evaluate the influencing factors of anxiety and depression status.Results The incidences of anxiety and depression in DR group(Anxiety:47.42%;Depression:47.42%)were higher than that in NDR group(Anxiety:10.94%;Depression:10.94%),and the differences were statistically significant(anxiety:?~2=13.049,P<0.001;depression:?~2=13.049,P<0.001);and the scores of anxiety and depression in DR group were higher than those in NDR group,and the differences were statistically significant(anxiety:t=-6.628,P<0.001;depression:t=-4.599,P<0.001).By univariate analysis,gender,age,time of diabetes,monthly income,understanding of their own condition,understanding that diabetes can cause retinopathy,visual acuity,and ophthalmic examination were the influencing factors of anxiety scores in diabetic patients(t/F=-3.795,4.364,-7.079,5.213,3.117,4.702,20.793,5.087,P<0.05).Gender,time of diabetes,monthly income,understanding that diabetes can cause retinopathy,visual acuity,and ophthalmic examination were the influencing factors of depression scores in diabetic patients(t/F=-2.363,-5.162,3.844,2.707,12.638,0.011,P<0.05).The results of multiple linear regression showed that gender,age,understanding of their condition,and visual acuity were the influencing factors of anxiety scores in diabetic patients.Among them,gender for female and understanding of their condition were positively related to anxiety scores,while age and visual acuity were negatively related to anxiety scores.Gender and visual acuity were the influencing factors of depression scores in diabetic patients.Among them,gender for female was positively related to depression scores,while visual acuity were negatively related to depression scores.Conclusion Patients with diabetic retinopathy had higher levels of anxiety and depression than those without retinopathy.Female patients,aged 45-60,with diabetes for more than 5 years,monthly income less than 2000 yuan,high level of understanding of the disease,and poor eyesight had higher anxiety and depression scores.Therefore,the clinical worker and caregivers need to pay attention to the emotional problems of diabetic patients,especially the female patients with retinopathy,long course of disease,low income and obvious vision decline,so it is necessary to give targeted intervention to improve the quality of life of patients.
