Objective:to observe the effect of scalp acupuncture combined with rehabilitation training on motor function,intestinal flora and inflammatory factors in patients with ischemic stroke,in order to explore the specific mechanism of this therapy on intestinal flora and inflammatory response in patients with ischemic strokeMethods:ninety patients with ischemic stroke who met the inclusion criteria were randomly divided into three treatment groups:scalp acupuncture group,rehabilitation training group and scalp acupuncture combined with rehabilitation training group.At the same time,30 healthy volunteers were recruited as control group.The patients in the group were given routine department treatment and acupuncture at body acupoints(selected Jianliao,Quchi,Zhouliao,Yanglingquan,Weizhong,Tianshu,Zhongwan and other acupoints),and the poles of the pulse therapeutic apparatus were connected with the needle handle after obtaining qi.The patients were treated with electro-acupuncture for 30 minutes twice a day to adjust to the stimulation intensity that the patients could tolerate.In the scalp acupuncture group,the apical area and the anterior parietal area were selected for cluster acupuncture,and the pulse therapeutic apparatus was connected with the needle handle after obtaining qi,and the electro-acupuncture treatment was performed once a day for 30 minutes.The acupuncture was retained for 8 hours after electro-acupuncture treatment,and the rehabilitation training group was given PT and OT training for 30 minutes each time,once a day,according to the needs of the patient's condition,and the scalp acupuncture combined with rehabilitation training group was treated on the basis of the above routine treatment.Rehabilitation training was carried out during the long needle retention period of scalp acupuncture for 30 minutes each time,once a day.The above treatment was performed 6 times a week for a total of 14 days.The simplified Fugl-Meyer motor function score(Fugl-Meyer Assesment,FMA),16s rRNA intestinal microflora analysis,serum TNF-a and MCP-1 content were selected.All data are analyzed and processed by SPSS21.0 softwareResults:1.FMA score:the simplified Fugl-Meyer score of patients after treatment was significantly higher than that before treatment.Compared with the three groups after treatment,the simplified Fugl-Meyer score of the scalp acupuncture combined with rehabilitation training group was significantly higher than that of the control group,and the difference was statistically significant.(P<0 05)2.Intestinal microflora analysis:compared with the healthy control group,the content of Lactobacillus decreased and the content of Clostridium increased in patients with ischemic stroke.The content of Lactobacillus in patients before and after treatment was lower than that in healthy control group and the content of Clostridium in patients before and after treatment was significantly higher than that in healthy control group(P<0.05).Compared with before treatment,the content of Lactobacillus increased and the content of Clostridium decreased after treatment,after treatment,the content of Lactobacillus increased and the content of Clostridium decreased in the three groups.The content of Lactobacillus increased and the content of Clostridium decreased significantly in the scalp acupuncture combined with rehabilitation training group,and the difference was statistically significant(P<0.05)3.The levels of TNF-? and MCP-1 in serum of patients with ischemic stroke were significantly higher than those of healthy controls,and the contents of TNF-? and MCP-1 in serum of patients with ischemic stroke before and after treatment were significantly higher than those of healthy controls.The difference was statistically significant(P<0.05).Compared with those before treatment,the contents of TNF-? and MCP-1 in serum of the three groups decreased after treatment,and the contents of TNF-? and MCP-1 in the patients of scalp acupuncture combined with rehabilitation training group decreased more significantly after treatment.The difference was statistically significant(P<0.05)Conclusion:1.Scalp acupuncture,rehabilitation training and scalp acupuncture combined with rehabilitation training can restore the motor function of patients with ischemic stroke,increase the content of intestinal Lactobacillus,reduce the content of Clostridium,and down-regulate the level of pro-inflammatory factors in serum2.In improving motor function,intestinal flora and inflammatory response in patients with ischemic stroke,scalp acupuncture combined with rehabilitation training is superior to simple scalp acupuncture and rehabilitation training.
