PurposeIschemic stroke is a cerebrovascular disease areas, that is, cerebral infarction, including atherosclerotic thrombotic cerebral infarction, cerebral embolism, lacunar infarction and watershed infarction. Cerebrovascular disease morbidity, mortality and disability were higher for the three major causes of death of human beings, of which about 75% of cerebral infarction. Cerebral ischemia, irreversible neuronal damage occurs, in 3 weeks about 90% of patients will occur limb spasticity, to the patients themself, their family and the society a great burden. Therefore, the treatment of acute illness on the prognosis and outcome of patients plays a crucial role, is to reduce mortality and reduce the degree of disability the key. In recent years, head for the treatment of the disease have yielded satisfactory results. Studies have shown that various types of head for the stroke and the therapeutic effect of both, and the scalp treatment as soon as possible involved in the acute phase, helps to improve the prognosis of patients with the disease. Scalp of the subject of international standardization of treatment programs, through clinical and experimental research, observation of the scalp acupuncture group and the rehabilitation of neurological function before and after treatment, Barthel index of clinical efficacy and comparison of difference; head for each time point of acute cerebral ischemia Neuronal deficits, brain tissue p38 mitogen-activated protein kinase (p38MAPK) expression, both from clinical and experimental aspects of scalp acupuncture treatment of acute ischemic stroke clinical efficacy and possible mechanism.MethodsClinical study:80 patients with acute ischemic stroke patients are the first hospital in Wuhan acupuncture and neurology patients. They were randomly divided into the scalp acupuncture group and rehabilitation group,40 patients in each group, with a course of 28 days, two groups of patients before and after the treatment of neurological deficits, Barthel index and clinical outcomes were compared. Experimental study:70 healthy SD rats were randomly divided into sham operation group, model group, scalp acupuncture group. Middle cerebral artery by intraluminal middle cerebral artery occlusion (MCAO) and reperfusion model, and scalp treatment, observed at each time point of acute cerebral ischemia reperfusion rat model and head for the neurological deficit, brain tissue and promote p38MAPK rats.Results1. Clinical study:(1) recovered after treatment with scalp acupuncture group and the group of patients with neurological deficit score (NSS) were decreased compared with before treatment, the difference was significant (P<0.05), and the therapeutic effect of scalp acupuncture group was significantly better than the rehabilitation group (P<0.01). (2) Barthel index score improved in both groups, and scalp acupuncture group improved on the Barthel index was superior to the rehabilitation group (P<0.01). (3) scalp acupuncture group and.rehabilitation group, rates were 87.5% and 67.5%, scalp acupuncture group was significantly higher than the rehabilitation group (P<0.05).2. Experimental study:acute cerebral ischemia neurological deficits:scalp acupuncture group and model group at each time between the two groups on the first 72h NSS scalp acupuncture group were significantly lower than model group (P<0.01). (2)p38MAPK expression:The Western bloting and RT-PCR detection of cerebral ischemia and 60min reperfusion 24h,48h and 72h, p38MAPK in infarcted brain tissue were expressed, and peaked at 24h in, but the first acupuncture group was significantly lower than model group (P <0.05).Conclusion1. Scalp acupuncture can alleviate acute ischemic stroke in patients with neurologic impairment and improve the daily lives of patients and cognitive ability, superior clinical efficacy of rehabilitation training.2. Scalp acupuncture will help to improve neurological function of cerebral ischemia, its mechanism of action against cerebral ischemia by inhibiting the expression of p38MAPK, thereby reducing the cerebral ischemia-reperfusion injury, to achieve the protective effect of brain tissue.
