Clinical Observation Of Danggui Yinzi And Guizhi Decoction Combined With Ebastine Tablets In The Treatment Of "Night Attack" Chronic Urticaria

Objective: This project includes two parts: epidemiological investigation and clinical observation.The purpose is: 1.To further investigate the time regularity and clinical symptoms of chronic urticaria from the perspective of epidemiology;2.To test the therapeutic effect of Danggui Yinzi and Guizhi Decoction combined with Ebastine Tablets in the treatment of "nighttime attack" chronic urticaria,and to verify the circadian rhythm of chronic urticaria.The rationality of guiding TCM dialectical thinking.Methods:1.To develop a questionnaire on the onset time of chronic urticaria and related complications,70 eligible patients were investigated by questionnaire.Questionnaire content includes basic information of patients,medical history,related concurrent symptoms and so on.At the end of the study,relevant data were collected and analyzed,and the research results were discussed in TCM theory combined with the clinical experience of tutors.2.Sixty-four patients with chronic urticaria with good nighttime compliance were included in the study.They were randomly divided into treatment group and control group,32 cases.The treatment group was treated with Danggui Yinzi and Guizhi Decoction,and treated with Ebastine tablets.Treated with ebastine tablets alone,the course of treatment was 4 weeks.The patients with MILAR,DLQI,total effective rate and adverse reactions were observed before and after treatment.The recurrence of the two groups was followed up after 8 weeks.Results:1.65.71% of the patients often attack at night,only 8.57% of the patients often attack during the day,and 25.72% of the patients have no circadian rhythm;78.57% of patients have seasonal rhythm,predisposed to summer and winter;this disease predisposes to women;concurrent symptoms are sleep disorders,fear of cold or heat,skin burning,dry mouth,fatigue,etc.nearly half of patients have negative emotions;the location of the disease is irregular.2.After 4 weeks of treatment,the total effective rate of the treatment group and the control group were 90.32% and 70.00%,respectively.After 8 weeks of treatment,the follow-up showed that the recurrence rate of the treatment group was 33.33%,and the recurrence rate of the control group was 68.75%.The difference in recurrence rate was statistically significant(P<0.05).The decrease in MIRR score and DLQI score in the treatment group was greater than that in the control group,and the difference was statistically significant(P<0.05).No adverse events occurred in either group during the treatment period.Conclusion:1.Most of the chronic urticaria attacks have diurnal rhythm and season,which occur at night,summer and winter.It is suggested that the pathogenesis of this disease may be related to human time rhythm.Based on the results of the investigation,we believe that Yingwei deficiency and nighttime predisposition of chronic urticaria are common pathogenesis.2.The curative effect of traditional Chinese medicine combined with Ebastine Tablet in treating chronic urticaria with nighttime attack guided by circadian rhythm of onset is better than that of Ebastine Tablet alone,and the recurrence rate is low.It is worth popularizing in clinical practice.
