Objective:Breast cancer is one of the main leading causes of death in women.Younger breast cancer patients(BCYW)are more aggressive,have more lymph node involvement,have a higher degree of malignancy,and have a worse prognosis than those in elderly patients.They are prone to lymph node metastasis,and the degree of malignancy is high.The aim of this study is to investigate the clinical and pathological features,such as the pathological types,the tumor size,the lymph node involvement and clinical expression of common immunohistochemical markers and molecular pathological features in young breast cancer patients.It provides a clinical pathological basis for individualized precision treatment to improve the survival rate and quality of life of young breast cancer.Methods:A retrospective analysis from January 2015 to February 2019 of 493 patients with Invasive breast cancer(including invasive ductal carcinoma,invasive lobular carcinoma,medullary carcinoma,mucinous adenocarcinoma and metaplastic cancer)in the Second Affiliated Hospital of Shanxi Medical University.Pathological data were used to select 70 young women(?40 years old)with breast cancer as the observation group.At the same time,125 elderly women(>60 years old)with breast cancer were selected as the control group.The two groups were selected the clinical and pathological data,analyzed the pathological types,tumor size,lymph node involvement,histological grade,and clinically used immunohistochemical markers(ER,PR,Her-2,Ki67)performed the molecular typing,and then tested them by chi-square test.Results:The pathological type and tumor size of young breast cancer patients were not significantly different from those of the elderly control group(p>0.05).While the lymph node involvement and histological grade of young breast cancer patients were higher than the control group(P<0.05).The positive rate of ER and PR expression in young breast cancer patients was lower than that in the elderly control group(P<0.05).But the positive rate of Her-2 and Ki67 expression was not significantly different f in the two groups(p>0.05).The molecular typing distribution ratios of the two groups were different.The young breast cancer patients with luminal type A,luminal type B,Her-2overexpressing,and Basl-Iike type,accounting for 7.1%,61.4%,21.4%,and 10%,while12%,72%,6.4%,9.6% in the elderly control group;the proportion of Her-2overexpression in young breast cancer patients was significantly higher than that in the elderly control group(P<0.05);Luminal breast cancer(including Luminal type A breast cancer and Luminal type B breast cancer),and the difference was statistically significant(P<0.05).Conclusion:Young breast cancer patients havemore lymph node involvement,lower degree of differentiation,lower positive expression rate of ER and PR,and higher proportion of molecular type Her-2 overexpression.So as to more invasive and poor prognosis than the elderly breast cancer patients.
