OBJECTIVE:This study reviewed 6847 newly diagnosed lung cancer patients from the Second Affiliated Hospital of Zhejiang University School of Medicine between December 2011 and December 2016,and screened a total of 199 lung cancer lesions from 87 cases of multiple primary lung cancers for retrospective analysis.The clinical characteristics of this kind of disease are described to explore the potential prognostic-related factors.METHODS:The clinical data and follow-up results of 87 patients with multiple primary lung cancers in our hospital were collected.The data includes clinical information,pathological classification and so on.The follow-up methods includes checking medical records from the outpatient department and telephone follow-up.Calculate the overall survival and recurrence-free survival.Kaplan-meier univariate analysis and COX proportional hazard regression model multivariate analysis are performed on factors that may be associated with prognosis.The difference of survival function between two groups is analyzed by log-rank test.P<0.05 is considered statistically significant.RESULTS:The incidence of multiple primary lung cancer in this study is 1.3%.Of the 87 patients,76 cases(87.4%)have simultaneous multiple primary lung cancers,58(66.7%)cases are female patients,and 81cases(93.1%)have adenocarcinomas,and 10 cases(43.8%)have EGFR positive alterations.For the highest stage in each case,stage I patients accounts for 74.7%.10 cases were lost to follow-up during the follow-up period,with a median follow-up of 46 months.Unilateral analysis of RFS in the contralateral multiple primary lung cancer VS ipsilateral multiple primary lung cancer patients is 97.6 VS 63.1(months),P=0.008.Patients combined with other tumor history could be a potential risk factor,but there is no significance.Factors such as age,gender,and history of severe smoking are not found to be significantly associated with recurrence-free survival.The acinar pathological subtypes of the simultaneous multiple primary lung adenocarcinomas accounted for the highest proportion(54.5%).Conclusion:The common pathological type of multiple primary lung cancer in this study is adenocarcinoma.Simultaneous multiple primary lung cancer and female patients are more common.The ipsilateral distribution may be a poor risk factor for multiple primary lung cancer.
