Objective:Multiple primary lung cancer is a rare clinical disease,accounting for less than 1% of all lung cancer patients,but its incidence increased year after year,has become an important threat to people's physical and mental health.This article discusses clinical characteristics of multiple primary lung cancer and its rational diagnosis and treatment.Methods:The clinical data of the Second Hospital of Jilin University in January 2010 to January 2016 were treated 9 patients with multiple primary lung cancer patients were retrospectively analyzed,Combined with the relevant literature materials,given a relatively scientific and reasonable incidence of the law and its clinical treatment.Our hospital received a total of 1071 cases of primary lung cancer,of which the diagnosis of multiple primary lung cancer in 9 cases,accounting for about 0.84% of patients with lung cancer,Male patients in 7 cases,2 cases of female,aged 43 to 76 years,the average age of 60 years;8 patients had varying degrees of smoking history,5 patients had a history of chronic bronchitis,1 patient had a family history of cancer;Foci located in the same side of the lungs of different leaves,2 cases,located in bilateral lungs in 7 cases;9 cases of multiple primary lung cancer patients,2 patients had no obvious clinical symptoms,only mild chest discomfort,due to physical examination found lesions admitted to hospital,7 patients due to varying degrees of irritating cough,sputum bloody,etc.Typical clinical symptoms and hospitalized for treatment;2cases were confirmed by pathology of fiberoptic bronchoscopy.The specific pathological findings were: 1 case of poorly differentiated squamous cell carcinoma and 1 case of adenocarcinoma with right differentiation of squamous cell carcinoma.The pathological results were obtained in 7 cases,the pathological combination,the right middle differentiated adenocarcinoma plus the left well differentiated adenocarcinoma in 2 cases,the right upper lobe,the middle adenocarcinoma in 1 case,the right poorly differentiated adenocarcinoma plus the left.Adenosquamous carcinoma in 1 case,the right side of the poorly differentiated squamous cell carcinoma with left poorly differentiated adenocarcinoma in 1 case,the right side of the well-differentiated squamous cell carcinoma of the left upper lobe of the well-differentiated adenocarcinoma of the left lower lobe of the poorly differentiated adenocarcinoma in 1 case,adenocarcinoma in situ on the left small additional invasive adenocarcinoma in 1 case.Resulths:Two patients were admitted to the hospital after treatment,respectively,8 months after discharge,13 months died,7 patients underwent surgical treatment,5 patients received adjuvant chemotherapy,including recurrence and systemic metastasis in 2 cases,after surgery 9,14 months,died,and the remaining five cases were normal survival.It should be noted that seven cases of surgically treated patients with multiple primary lung cancer,lung cancer was confirmed as early as many as four cases,lesion diameter usually less than 1cm,after 5-year survival rate was 100%,almost the purpose of healing.Conclusion:The incidence of multiple primary lung cancer is low,mainly in older men,but is younger,pathological type of adenocarcinoma and its related types;Diagnosis of the proposed selection of high-resolution CT additional fiber bronchoscopy,the diagnosis of early foci,especially the diagnosis of pulmonary nodules on the late treatment is of great significance;Early surgical resection is better,almost to achieve the purpose of cure.
