Objective:To discuss the clinical characteristics and prognostic factors of patients with multiple primary malignancies(MPM)involving lung cancer.To improve the clinical understanding of multiple primary malignancies involving lung cancer,and try to find some clues for early diagnosis ang detection,prevention and clinical treatment of these patients.Methods: The clinical data of 174 cases with MPM involving lung cancer from January 1,2003 to April 30,2016 in our hospital were analyzed retrospectively,which included interval time,histological type of lung cancer,lung cancer stage,sites of tumors,and therapies.Results: Among 174 patients with MPM involving lung cancer,167(96.0 %)had two malignancies and 7(4.0 %)had three malignancies.67(38.5 %)were lung cancer first MPM patients and 107(61.5 %)were other cancer first MPM patients;46(26.4 %)were synchronous MPM patients and128(73.6 %)were metachronous MPM patients.the second primary lung cancer and upper respiratory cancer(28.2%)was the most common type of primary malignancies in other organs,followed by digestive system tumors(25.4%),Urogenital system tumors(17.1%),breast cancer(15.5%)and so on.Patients with MST were 66 months,1,3,5 year survival rates were 89.6%,72.4%,56.8%;Different lung cancer staging patients,The OS of stage ?-? a(n =107,61.5%)was significantly longer than that in stage ?b-?(n = 67,38.5%)(P<0.001).The number of OS in the Lung cancer first group(n = 67,38.5%)was significantly lower than that in the non-lung cancer first group(n = 107,61.5%)(P =0.002);synchronous MPM group(n = 46 cases,26.4%)had a significant prolongation(P <0.001)in the metachronous MPM group(n = 128 cases,73.6%);The radical treatment group(n = 86,49.4%)had significantly prolonged OS(n = 88,50.6%)in the palliative treatment group(P =0.002).Scope of Cox model,multi-factor analysis showed that lung cancer stage,lung cancer occurrence,synchronous or metachronous,lung cancer treatment are the independent factors affecting Overall Survival Time with MPM involving lung cancer.Conclusion: 1.In MPM involving lung cancer,double primary malignancies are more common than others,other cancer first MPM are more than lung cancer first MPM,metachronous are more than synchronous MPM involving lung cancer;2.Lung cancer patients prone to recurrent respiratory system,digestive system malignant tumor;Recurrent lung cancer with digestive system,genitourinary system,respiratory system,breast cancer patients with malignant tumors more common;the second primary lung cancer and upper respiratory cancer was the most common type of primary malignancies in other organs,followed by digestive system tumors,Urogenital system tumors,breast cancer;3.Lung cancer stage,lung cancer occurrence,synchronous or metachronous,lung cancer treatment are the independent prognostic risk factors with MPM involving lung cancer.
