| Purpose:Through the detection of chronic obstructive pulmonary disease with acute exacerbation(Acute Exacerbation Chronic obstructive pulmonary disease,AECOPD)patients before and after treatment,changes of blood uric acid,uric acid/creatinine values,and according to the severity of AECOPD before treatment serum uric acid levels,and to analyze the correlation between uric acid and oxygenation index,AECOPD patients with reaction the condition changes of uric acid,uric acid and correlation with the degree of hypoxia,so the uric acid,uric acid/creatinine changes and clinical significance in monitoring the disease activity of AECOPD.Method:Review from November 2015 to December 2016 in Sir Run Run Shaw Hospital affiliated to Zhejiang University after hospitalization were discharged after the acute exacerbation of chronic obstructive pulmonary disease(AECOPD)in 98 patients as the experimental group,the anti-inflammatory and antiasthmatic,expectorant and oxygen therapy were cured;collect the same age 50-85 years old,the same period in the medical examination center of Sir Run Run Shaw Hospital health examination 98 healthy cases as the control group;collect the same age 50-85 years old,the same period in Sir Run Run Shaw Hospital outpatient visits up in March did not occur within 98 stable COPD patients with acute exacerbation of the cases,all of the subjects were excluded from malignant tumor,liver and kidney dysfunction,connective tissue disease,diabetes,coronary heart disease and other diseases.Patients in the experimental group were collected within 24 hours of admission blood uric acid(UA),creatinine(Crea)and arterial blood gas,and after 5-21 days of treatment,the improvement of venous blood UA,Cr and arterial blood gas.Comparison of the experimental group before and after treatment,stable period,the control group between serum uric acid and uric acid/creatinine changes,and according to the severity of AECOPD before treatment serum uric acid,uric acid and oxygenation index correlation analysis using linear regression,and multi factor design:uric acid(UA,moi/L)as the dependent variable(A,BMI,B,C,D group),group(AECOPD group and healthy control group)as fixed factors(independent variables),analysis of uric acid changes in both of them influence the situation.P<0.05 was statistically significantResult:AECOPD patients before UA,UA/Cr values were significantly higher than the control group after treatment,and the stability of group UA,UA/Cr;UA,UA/Cr value is higher than the stable group after treatment UA,UA/Cr;UA,UA/Cr after treatment was lower than the control group UA,UA/Cr;the P<0.05 were compared,the differences were statistically significant.Compared with control group UA,UA/Cr,UA,UA/Cr,the difference was not significant in stable group,P>0.05.The severity of III in patients with AECOPD before treatment was significantly higher than that of patients with ?,P<0.05,the difference was statistically significant.Because of the variable Y1 in the treatment of uric acid before before treatment,the oxygenation index as independent variables X1 linear regression analysis,the Pearson correlation coefficient of R is-0.889,R is 0.790,there is a negative correlation between them,and the close relationship between the linear regression equation:Yl =-1.42 X1+663.28.Multivariate analysis showed that uric acid in the AECOPD group and the control group were significantly different,P<0.05;BMI in each grade was no significant difference between the P>0.05;the interaction between the two was not significant,P>0.05.Conclusion:COPD at the acute stage of uric acid and oxygenation index was negatively correlated with the body,so the degree of hypoxia was positively correlated with serum uric acid,uric acid/creatinine level changes will herald the prognosis in patients with AECOPD,should pay attention to uric acid,creatinine and uric acid/AECOPD relationship,to assess the severity of illness have a certain value,can be used as one of the indicators for monitoring the disease is simple,convenient and economical. |
