| ObjectiveTo observe the therapeutic effect of needle knife on patients with knee osteoarthritis,and to explore the outcomes of needle knife on symptoms which were evaluated by JOA knee score and gait parameters measured by gait analysis.MethodsSixty patients with knee osteoarthritis who met the diagnostic and inclusion criteria were randomly divided into needle knife group(treatment group)and electroacupuncture group(control group).In the needle knife group,6 sites were chosen for the operation including the upper and lower medial collateral ligament of the knee,the upper and lower lateral collateral ligament,the stimulation points of the inferior patellar ligament,and the suprapatellar capsule of the affected side were taken from the operative sites.The patients were treated with needle knife once every two weeks,with one course of treatment at 4 weeks and one course of treatmc nt observed.Electric acupuncture group take Xue Hai,Liang Qiu,Nei xiyan,Wai xiyan,He Ding,Yang Lingquan,Yin Lingquan,Zu Sanli,needle acupuncture needling with G6805-Iin the above the needle,electric,respectively for 2/100 HZ frequency density wave,treatment 3 times a week,four weeks for a period of treatment,to observe a period of treatment.JOA score,efficacy evaluation and plantar pressure were used as the observation indexes.Through observation and comparison of the corresponding observation indexes recorded in the treatment course of the two groups of patients,the resul ts were discussed and analyzed after statistical collation.Results1.Before treatment,the basic information of the two groups of patients included gender,age,course of disease,number of knces,VAS pain score,JOA score,knee score,step Angle and other baseline data for comparison.The difference between the two groups was not statistically significant(P>0.05),which was comparable.2.The mean value of VAS after 2 weeks of treatment was lower than that before treatment,and the mean value of VAS after 4 weeks of treatment was lower than that after 2 weeks of treatment,with statistically significant difference(P<0.001).The results indicated that pain was significantly improved in both groups after 2 weeks and 4 weeks of treatment.Moreover,the improvement of pain in the needle knife group after 2 weeks and 4 weeks of treatment was better than that in the electroacupuncture group,and the difference in treatment was statistically significant(P<0.05).3.All items JOA score before treatment and after treatment was used,and after the treatment was improved,the difference was statistically significant(all P<0.05),suggesting that the needle knife and cupping treatment of KOA patients suffering from knee function has improved,and two sets of all items JOA score by independent sample t test,the difference was statistically significant(all P<0.05),that is,two groups of patients after treatment,needle knife group of all items of JOA symptoms improvement were superior to electric acupuncture group.4.After treatment and before treatment,both groups showed improvement in step Angle,and the difference was statistically significant(P<0.001).After treatment,the average step Angle of needle knife group was higher than that of acupuncture group.In addition,t test of two groups of independent samples showed that the improvement of step Angle in needle knife group was better than that in electroacupuncture group,with statistical difference(P<0.05).5.Total clinical efficacy evaluation,total effective rate was 98.00%,excellent in 2 cases,good in 33 cases,medium in 5 cases,poor in 1 case;There were 38 cases in the electroacupuncture group,1 excellent,8 good,25 medium and 5 poor.The rank-sum test showed that there was a statistical difference between the two groups(P<0.05),suggesting that the efficacy of needle knife in the treatment of knee osteoarthritis was better than that of the electroacupuncture group.Conclusion1.needle knife can effectively relieve pain and dysfunction of patients with osteroarthritis,correct long-term spasm and contracture of the soft tissue around the joint,and restore its mechanical balance through the release of related muscles,ligaments and other soft tissues.Compared with control group,treatment group has more significant efficacy,shorter course of treatment and lower cost,and is worthy of further clinical application.2.The changes of gait characteristic indexes of patients with KOA before and after treatment had a positive correlation with the improvement of their symptoms,which could provide a mechanical reference for KOA’s efficacy. |
