ObjectiveEndometriosis is a common and frequently-occuring disease in gynecology,and its main clinical symptom is dysmenorrhea.Dueing to long-term chronic pain,women who suffering from this disease have harmful physiological,psychological and social impacts.It also brings a heavily financial burden to patients,their families and society,which is the dilemma faced by the world today.Moreover,the pathogenesis of endometriosis is not completely clear,and its condition repeatedly manifests suddenly,is difficult to cure completely.At present,the main treatment methods include surgical treatment and conservative treatment.Among them,conservative treatment is divided into western medicine treatment and traditional Chinese medicine treatment and traditional Chinese Medicine treatment with Characteristics Therapy.At present,many doctors have their own opinions and experiences on acupuncture and moxibustion treatment of this disease,and have achieved a certain effect,and the treatment of acupuncture and moxibustion is highly praised by doctors and patients because of its high safety and few side effects.But the current acupuncture treatment only considers the simple treatment,does not consider the time pertinence of dysmenorrhea,not combining with ChronoMedicine.This study combines acupunct uretherapy with ChronoMedicine,exploring the clinical efficacy and safety of Najia method of midnight-noon ebb-flow acupuncture in treating endometriosis—induced dysmenorrhea.This study has certain practical significance and can provide a new way of thinking and treatment for this disease.MethodsBy a randomized controlled clinical trial,50 patients with dysmenorrhea(or adenomyosis)of qi stagnation and blood stasis type were selected from January 2017 to December 2017 in gynecological clinic of Guangdong Provincial Hospital of traditional Chinese Medicine.They were randomly divided into treatment group and control group.The patients in these two groups were treated by basic syndrome differentiation prescription(Zhongji(CV6),Diji(SP8),Zusanli(ST36),Ciliao(BL31),Sanyinjiao(SP6))inputting into the low-frequency midnight-noon ebb-flow therapeutic apparatus.The patients in observation group were given midnight-midday ebb flow acupuncture based on dysmenorrhea prescription,and the patients in control group were given false point based on dysmenorrhea prescription.The patients were treated once every 3 days within one week before menstruation,and once a day during menstrual period,one course of treatment included one menstrual cycle,all the patients were treated for three menstrual cycles.After treatment,follow-up was performed on the first day after end of menstruation.The scores of TCM symptoms,VAS of dysmenorrhea degree and EHP-5 scale were observed before,and after treatment and during follow-up in the two groups to evaluate the curative effect.Results1.The baseline data were compared between the two groups before treatment,including age,course of disease,marriage,VAS score of dysmenorrhea degree,main symptoms of TCM,secondary symptoms of TCM,total score of TCM symptom or each score item of EHP-5 scale.The difference was not statistically significant(P>0.05)and was comparable.2.In the aspect of reducing dysmenorrhea degree,the VAS score of dysmenorrhea degree in both groups decreased after treatment,and there was significant difference between the two groups before and after treatment(P<0.01).At follow-up,the VAS scores of dysmenorrhea degree in both groups were also decreased,comparing with end-of-treatment,the difference in the observation group has significant statistical difference(P<0.01),while the comparison of VAS scores in the control group was no statistical difference(P>0.05).Observing the VAS scores of dysmenorrhea at different time points in the two groups,the VAS scores of the observation group were obviously lower than those of the control group,the difference was statistically significant(P<0.01).The total effective rate was 92.00%in the observation group and 62.50%in the control group.There was significant difference between the two groups in total effective rate and curative effect grading(P<0.05).3.In the improvement of TCM symptoms,the scores of main symptoms,secondary symptoms and total scores of TCM symptoms in the two groups were obviously decreased after treatment,and there was significant statistical difference between the two groups before and after treatment(P<0.01).At follow-up,comparing with end-of-treatment,the scores of TCM symptoms in the two groups were still decreased.The scores of TCM symptoms in the observation group has significant statistical difference(P<0.01).There was no statistically difference in the scores of main symptoms in the control group(P>0.05),while the scores of secondary symptoms and total scores of TCM symptoms in the control group were statistically different(P<0.01).Observing the scores of total scores of TCM symptoms at different time points in the two groups,the total scores of TCM symptoms of the observation group were obviously lower than those of the control group,the difference was statistically significant(P<0.01).The total effective rate was 88.00%in the observation group and 58.30%in the control group.There was significant difference between the two groups in total effective rate and curative effect grading(P<0.05).4.In terms of EHP-5,After treatment,the mean value of all items in the two groups was significantly lower than that before treatment,and there was significant statistically difference between the two groups(P<0.01).For items 1,2,3,4,5,6 and 7,that is,pain affected walking,symptoms affected daily life,emotional instability,perceived incomprehensibility,impaired image,pain affected work,pain affected taking care of children and other improvements in quality of life,the difference was statistically significant(P<0.05),and items 8,9,10,11 of the comparison,namely,the patient reject sexual intercourse for fearing painess,unable to babysit children,feel that the doctor misread,fear of infertility,there was no significant difference between the two groups(P>0.05).At follow-up,comparing with end-of-treatment,the scores of each item in the two groups were still decreased,the difference was statistical(P<0.05).Compared the descending in the control group with those in the control group,items 1,2,6 and 7 had significant difference(P<0.05),while there was no statistically difference in the items 3,4,5,8,9,10,11(P>0.05).ConclusionThe Najia method of Midnight-noon ebb-flow combining with dialectical acupuncture therapy has advantages in relieving the degree of dysmenorrhea,reducing the score of TCM symptoms and improving the quality of life in patients with dysmenorrhea of qi stagnation and Blood stasis type,and the curative effect is better than that of simple dialectical acupuncture therapy,and it is not only safe but also simple.It is worth studying and popularizing further.
