The new rural cooperative medical care system is an indispensable part of China's basic medical security system.The range and funding standards of the new rural cooperative medical system have been continuously increased,but it is also accompanied by the waste of medical resource and the increase in medical expenses.Therefore,it is important to study the impact of the new rural cooperative medical system on the medical burden of farmers.It not only helps to understand the internal mechanism of the new rural cooperative medical system to release the medical service demand of farmers,but also provides a reference for the government to launch corresponding medical security policies.The main content and results of this paper are as followed:Firstly,this paper sorts out domestic and foreign literatures and related theories.The relevant literature content,such as the evaluation of the design effect of the new rural cooperative medical system,the feasibility and quality of medical services,the measurement and evaluation of medical burdens,etc.,were extracted.And the relevant theories such as welfare maximization theory,medical insurance theory,and Andersen medical service utilization behavior theory were adopted.Secondly,combined with the status quo of farmers' medical treatment under the new rural cooperative medical system,the source of medical burden is explained from the perspectives of demand-side behavior change and supply-side behavior change,and corresponding hypotheses were put forward.The hypothesis mainly analyzed the internal mechanism between the actual reimbursement ratio,the self-paid medical expenses,the proportion of self-paid medical expenses,the large medical expenditure and the new rural cooperative medical system.Thirdly,this paper used the cross-sectional data of "China Health and Pension Tracking Survey" for empirical analysis of 2015 to compare the differences in medical burden between participating residents and non-participating residents in diverse dimensions.The empirical results showed that the popularization of the new rural cooperative medical system has played a positive role in reducing the medical burden of rural residents,but the burden of high medical expenditure is still heavy,and especially the phenomenon of "returning poverty due to illness" in low-income families has not been effectively dismissed.The better the health of rural residents,the greater the risk of disease resistance;The actual reimbursement rate has been raised,and the actual total medical expenditure of farmers has been significantly reduced.What's more,individual characteristic variables such as male,widowed and separated,and older have also significantly affected the medical burden of individuals and families.Finally,based on the research results,relevant conclusions and countermeasures were proposed,including improving the compensation mechanism,reducing the incidence of unreasonable medical treatment projects;improving information transparency and controling moral hazard;improving the major disease security system,increasing the protection of low-income groups,increasing public health investment,and actively carrying out health education.
