Professor Wang Xixing's Experience In TCM Differentiation And Treatment Of Liver Metastasis From Colorectal Cancer Based On RWS

Objective : Based on the clinical real world medical record data of Shanxi Traditional Chinese Medicine Hospital,data mining methods such as frequency analysis,K-means clustering algorithm,principal component analysis(PCA),Apriori correlation analysis were used.To explore the inherent law between the differentiation of diseases,differentiation of symptoms and treatment of liver metastasis from colorectal cancer and summarize Professor Wang Xixing''s clinical experience in the treatment of liver metastasis from colorectal cancer.Methods:This study belongs to the clinical application research.According to the theory of real world scientific research methodology,using the data mining platform of Wang Xixing's famous doctor studio,the collected medical records are input into the data mining system of Wang Xixing's famous doctor studio,and all data are processed standardizatively,and the data are checked again.In the way of "man-machine integration,human-centered",all data are analyzed by professional data mining experts using a variety of data mining methods,and the best results are selected according to Professor Wang Xixing himself and experts from famous medical studio.Using frequency analysis,K-means clustering algorithm,principal component analysis(PCA),Apriori correlation analysis and other statistical analysis methods,statistics of prescriptions used,summarized and analyzed Professor Wang Xixing's prescriptions for treatment of liver metastasis from colorectal cancer rules and experience.Results:1.1.Distribution characteristics of outpatient patients: female patients are more than male patients;patients aged 46-75 years are the majority;the proportion of primary site in colon cancer and rectal cancer is basically the same;patients who choose combination of Western medicine treatment are significantly higher than patients treated with pure traditional Chinese medicine.2.The common symptoms of liver metastasis from colorectal cancer are loose stool,poor sleep,heavy anus,fatigue,dry mouth,poor appetite,liver pain,abdominal distention,bitter mouth,etc.3.K-means clustering algorithm was applied to all symptoms,and the sum of squares of errors was the smallest when K value was 4 clusters,seeds were 30,and the maximum iteration was 500 times.Cluste0(5.27%),Cluste1(70.83%),Cluste2(13.13%),Cluste3(10.77%)?4.Principal Component Analysis(PCA)was used to analyze each cluster of symptoms.The main symptoms of Cluste0: nausea,vomiting,belching,poor appetite,loose stool,pulse chord.The main symptoms of Cluste1 were fatigue,heavy anus,poor appetite,abdominal distension,loose stool and weak pulse.The main symptoms of Cluste2 were fullness of stomach,vomiting,nausea,dry mouth,bitter mouth and loose stool.The main symptoms of Cluste3 were bloating in the liver area,pain in the liver area,irritability,poor appetite,loose stool and weak pulse.5.Comparing with the original data,the main syndromes and corresponding principal parties of each cluster are sorted out.Cluste0: Spleen and Stomach Disharmony Syndrome: Yiqi Jianpi,Hewei Sanjie,Liujunzi Decoction plus or minus application.Cluste1: Spleen Deficiency and Qi Depression Syndrome: Bupi Yiqi,Shengyang Lift Depression,Buzhong Yiqi Decoction plus or minus application.Cluste2: Miscellaneous Syndrome of Cold and Heat: Harmonizing Cold and Heat,Eliminating Expansion and Dispersion,Application of Banxia Xiexin Decoction.Cluste3: Liver and spleen disorders: soothing liver and relieving depression,nourishing blood and invigorating spleen,Xiaoyao powder plus or minus application.6.Analysis of commonly used medicines: Summarize the medicines used more than 50% in all prescriptions: Fritillaria thunbergii,Cistanche mushroom,centipede,Shijianpan,Bupleurum chinense,Angelica sinensis,Astragalus membranaceus,Dangshen,Atractylodes macrocephala,Citrus peel and Cistanche.Drug properties are mainly warm,followed by cold,mild cold,and then plain drugs.Drug taste is mainly sweet,bitter and bitter.Common medicines are mostly attributed to the spleen,liver,lung and stomach meridians.Its efficacy is mostly for invigorating qi,clearing away heat and detoxification,relieving exterior symptoms,nourishing blood,extinguishing wind and tonifying yin.7.Analysis of drugs and horn medicine: After removing the main prescription of syndrome differentiation,there were four groups of drugs with more than 90%confidence: Ophiopogon japonicus-Schisandra chinensis,centipede-Shijianpan,Fritillaria thunbergii-Cigus mushroom,tulip-curcuma;one group with more than 90%confidence: Ligustrum lucidum-Dry Lotus-Schisandra chinensis.Conclusion : Professor Wang Xixing differentiates and treats the common syndromes of liver metastasis of colorectal cancer: spleen deficiency and qi depression(70.83%),cold-heat miscellaneous syndrome(13.13%),liver-spleen disorder(10.77%)and spleen-stomach disharmony(5.27%).Common pairs of medicines: Ophiopogon japonicus-Schisandra chinensis,centipede-Shijianpan,Fritillaria thunbergii-Cigus mushroom,tulip-turmeric;Common corner medicines:Ligustrum lucidum-Dry Lotus-Schisandra chinensis.Drug properties are mainly warm,followed by cold,mild cold,and then plain drugs.Drug taste is mainly sweet,bitter and bitter.Common medicines are mainly attributed to the spleen,liver,lung and stomach meridians.The pathogenesis is deficiency of both liver and spleen,with phlegm and toxin joining each other locally.
