Objectives(1)This research aims to study the relationship between disease knowledge level and self-management behavior in community coronary heart disease patients in Wuhan.(2)Through strengthening and maintaining health education for patients with coronary heart disease in community,and the effects of health education on disease knowledge level,self-management ability,readmission rate and readmission rate were investigated.Methods(1)The convenient sampling method was applied in this research and 100 patients with coronary heart disease in a community in Wuhan were selected.The basic information of patients was obtained by distributing general information questionnaire and coronary heart disease related knowledge questionnaire was used to evaluate the disease knowledge level of patients.The self-management behavior scale was applied to evaluate patients' self-management ability and analyze the correlation between disease knowledge level and self-management behavior.(2)In this research,60 patients with coronary heart disease in a community in Wuhan were selected by method of convenient sampling,and their knowledge level was evaluated by the corresponding questionnaire.Coronary heart disease self-management behavior scale was used to assess patients' self-management ability and to monitor the readmission rate and readmission rate of patients.(3)Statistical method,the data were analyzed by spss20.0 software.The average data used in the general data was measured by mean number of standard deviation,the number of data was calculated by frequency and percentage,and the relationship between the related knowledge of coronary heart disease and the self management behavior was analyzed by Pearson correlation analysis,and before and after the intensive maintenance intervention and observation.The relative knowledge of coronary heart disease before and after the period was compared,and the self matched t test was used before and after the intensive maintenance intervention,and the readmission rate and the rate of revisiting rate before and after the intensive maintenance intervention were compared with the self matched 2 test.Results(1)In this research,106 questionnaires were sent out,100 valid questionnaires and 6 invalid questionnaires were collected.The questionnaire was found by statistical analysis.(1)Our research results indicated that the related knowledge of coronary heart disease of the patients was 37.57 ±6.58.(2)The average score for patients' self-management behavior was 73.90 ±12.14.(3)The correlation coefficient between coronary heart disease related knowledge and self-management behavior was 0.634%(P<0.001).(2)During the intervention,88 patients completed the evaluation of coronary heart disease related knowledge,68 patients with less than 42 scores were selected for intervention.During the intervention,8 patients lost their visits and 60 patients were finally completed.The results were as follows:(1)in the course of intervention,88 patients completed the evaluation of coronary heart disease(CHD)and 68 patients(less than 42 points)were involved in the intervention.After statistical analysis,the data were found(1)The total score of disease knowledge and the score of each dimension after intensive maintenance intervention were higher than those before intervention(P <0.05).(2)After observation period,the scores of coronary heart disease related knowledge from two dimensions of inducing factor and drug knowledge were lower than those before observation period(P<0.05),but there was no significant statistical difference in other dimensions(P>0.05).(3)The total score of self-management behavior and the score of each dimension after intensive maintenance intervention were higher than those before intervention.(P<0.05).(4)The readmission rate after intensive maintenance intervention was significantly lower than that before intervention.(P<0.05).(5)The revisiting rate of patients after intensive maintenance intervention was slightly lower than that before intervention,and the difference was not statistically significant.(P>0.05).Conclusions(1)The disease knowledge of patients with coronary heart disease in community is at the medium level.(2)Self-management behavior of patients with coronary heart disease in community is at the moderate low level.(3)There was a moderate positive correlation between disease related knowledge and self-management behavior in community coronary heart disease patients.(4)Strengthening and maintaining health education can effectively improve and maintain the disease knowledge level of patients with coronary heart disease in community,so as to improve the self-management ability of patients,reduce their readmission rate.At the same time,it has a positive effect on reducing the re-visiting rate of patients.
