On The Rule Of Treatment Of Bone Marrow Suppression After Chemotherapy Based On Data Mining

Purpose:To explore the syndromes and medication rules of traditional Chinese medicine(TCM)for the treatment of bone marrow suppression after chemotherapy in patients with cancer,in order to provide the reference and evidence for the clinical treatment and experimental study of myelosuppression after chemotherapy.METHODS:The cases of oncology and hematology in the case system of the First Affiliated Hospital of Guangzhou University of Traditional Chinese Medicine were searched,and the case information of tumor patients with myelosuppression after chemical drug treatment from June 2013 to June 2018 was collected.Excel 2013 software was applied to establish a post-chemotherapy bone marrow suppression data collection form and the syndrome information was input into it.The medical records without clear evidence,drug informat ion or other basic collection informat ion were excluded.And then classify and input the TCM syndrome and treatment information of patients with myelosuppress ion after screening.After collecting and sort ing,a total of 772 effective medical records were obtained,and a database of rules for the syndrome and treatment of bone marrow suppression after chemotherapy was established based on it.To standardize the processing of data,this study appl ied standards such as Chinese Medicine Symptom Diagnosis And Diagnosis(second edition),Chinese Medicine Diagnosis(thi rd edition),Chinese Medicine Internal Medicine(2011-2015 edition),People's Republic of China Pharmacopoeia(2015 edition),Chinese Medicine Dictionary and others.Descriptive analysis,factor analysis,association rule analysis and cluster ana lysis were applied on the syndrome and treatment information with the application of mainly R language software.Results:772 effective medical records were obtained after collecting and sorting,among which lung cancer was of the highest incidence rate,and mainly for myelosuppression degree I and II.40 high-frequency chemotherapy drugs and 20 chemotherapy regimens were obtained,among which the most used chemotherapy drug was cis-platinum.wh ich reached 237 times,and the highest frequency of chemotherapy regimen was DP.There were 34 stat i st ical high-frequency symptoms,wh ich inc luded 12 tongue and 12 pulse manifestations.The highest frequency of symptoms was insomnia,which was 199 times.The highest frequency of tongue was white tongue coating,which was 505 times with a frequency of 18.44%.The highest frequency of pulse was thin pulse,which was427 times with a frequency of 31.79%.By applyi ng factor analysis and associat ion rule ana lysis,10 symptom common factors and 44 syndrome associat ion rules were obtained.After inputt i ng 772 prescriptions,the top five most frequently used Chinese med icines were obta ined,which were Licor ice(Gan Cao,569 times),Pori a cocos(Fu Ling,464 times),Godonops is pilosula(Dang Shen,395 times),Bighead Atractylodes Rhizome(Ba i Zhu,371 times)and Pinell ia ternata(Ban Xia,281 times).The medicines were ma inly attributed to tonic med icine(yin-nourishing medicine,yang-enforcing medicine,blood-replenishing med icine,qi-inv igorating med icine),heat-clear ing medicine(heat-clear ing and detox ifying medicine,heat-clear ing and blood-cool ing med icine and heat-clearing and dampness-drying medicine),blood-act ivat ing and stasis-resolving medicine,and diuretic medicine,phlegm-resolving and cough-pant ing-relieving medicine and qi-regulat ing med icine.The medicinal nature ware ma inly mi ld,warm and cold,and the taste of the medicine were ma inly sweet,bitter and pungent.Their meridian entry were ma inly the lungs,liver,stomach,kidney,heart and spleen.The top three most used parts of the herbs were root,stem and rhizomes.The association rules of 33 herb pairs and groups were obta ined according to the association rule ana lysis,and 7 herb clusters were obta ined by cluster analysis.Conclusion:The result of myelosuppression after chemotherapy was mainly related to the plat inum-based chemotherapy med icine in the process of killing tumors.It consumed the physical resistance and resulted in patterns of deficient types combined with excessive pattern and also the intermingled deficiency and excess pattern.Deficiency pattern includes lung-spleen qi deficiency pattern,qi and blood deficiency pattern,liver and kidney deficiency,etc.;excessive pattern includes phlegm and blood stasis pattern,phlegm-dampness pattern,and phlegm-heat obstructing the lung pattern.intermingled deficiency and excess pattern includes liver depression and spleen deficiency pattern,spleen def iciency and dampness obstruction pattern,and yin deficiency with exuberant fire pattern,etc.The treatment of myelosuppression after chemotherapy is ma inly based on strengthening healthy qi and combining with the method of al leviat ing the pathogenic factors.The methods of restoring deficiency included re i nforcing qi to restore deficiency and tonifying qi and nourishing blood,and the method of alleviating pathogenic factors includes clearing heat and resolving tox in,activating blood to remove stas is,and dredging dampness and resolving phlegm.At the same time,it was combined with other methods such as promoting digestion and regulating qi,re lieving superficial a-spect and dredging dampness,pacifying liver and calming the mind,and warming yang to consolidating interior,etc.The top five high frequency herbs were Gan Cao,Fu Ling,Dang Shen,Bai Zhu,and Ban Xia in sequence,and the most frequently used herbs were deficiency tonifying herbs,reaching 28.53%,fol lowed by heat-clearing herbs,blood activating and stasis removing herbs,diuresis herbs,and phlegm resolving and cough-asthma relieving herbs.The properties of the herbs were mainly mild,warm and cold,and the taste of the herbs were mainly sweet,bitter and pungent.The meridian tropisms of the herbs were mainly to the lung,liver,stomach and kidney mer idian,and the medicinal parts were root and stem.
