Research On The Rule Of Prescription And Use Of Traditional Chinese Medicine In The Treatment Of Primary Nephrotic Syndrome By Data Mining

Objective : This study uses data mining methods to retrospectively analyze the medical records of the tutor Shi Junhua’s treatment of primary nephrotic syndrome,to explore Shi Junhua’s prescription and medication rules for the treatment of primary nephrotic syndrome,and to explore the treatment of primary nephrotic syndrome by Chinese medicine.The medication experience,ideas and methods of nephrotic syndrome provide reference for clinical treatment of primary nephrotic syndrome.Methods: The medical records of patients with primary nephrotic syndrome(from October 2018 to December 2020)who were diagnosed and treated by Chief Physician Shi Junhua in the inpatient department of the Department of Nephrology of Hubei Provincial Hospital of Traditional Chinese Medicine were collected.(1)Set the inclusion and exclusion criteria,and strictly screen 52 patients that meet the criteria,with a total of 234 prescriptions.Enter basic patient information and prescription drugs into Excel software,preprocess the data,and establish a database and data matrix.(2)Use Excel software to perform frequency analysis on the database to obtain the patient’s gender and age distribution.Shi Junhua’s high-frequency drugs for the treatment of primary nephrotic syndrome(manually set the drugs with a frequency of≥15 times as high-frequency drugs)and all it involves the distribution of the meridian efficacy of the drug’s nature and flavor.(3)Use IBM SPSS Statistics 25.0 statistical software to perform cluster analysis and factor analysis on the data matrix of high-frequency drugs to obtain core drug pairs and core drug groups in high-frequency drugs.After consulting Shi Junhua himself,we will analyze and discuss this.Results: 1.In this study,a total of 234 prescriptions were screened,involving 165 Chinese medicines,and the total frequency of medication was 3809 times.Among them,a total of 50 flavors were used ≥15 times,and the cumulative frequency was 3300 times,accounting for 86.64% of the total frequency.It is a high-frequency medicine used by Shi Junhua to treat primary nephrotic syndrome.There are 15 Chinese medicines with frequency of use ≥95 times and use rate(frequency of single medicine use/total number of prescriptions)>40%,accounting for 52.51% of the total frequency.They are Shi Junhua’s core drugs for the treatment of primary nephrotic syndrome.It is: Astragalus,Poria,Angelica,Tangerine Peel,Codonopsis,Yam,Bran-fried Atractylodes,Chixiaodou,Dogwood,Plantago Seed,Licorice,Salvia,Corn Silk,Rehmannia Root,Potbelly Bark.2.Among the 165 medicines in this study,the medicinal properties are mainly warm and flat,followed by cold;the medicinal taste is mainly sweet,bitter and pungent;the meridian is mainly spleen,lung,liver,kidney,heart and Stomach is the main one;the main functions are tonic drugs,diuresis and dampness drugs,heat clearing drugs,qi regulating drugs,blood promoting and stasis removing drugs,astringent reducing drugs,and phlegm relieving cough and asthma drugs.3.Among the 50 high-frequency drugs in this study,27 pairs of commonly used drugs were obtained,of which 8 pairs of core drugs were: golden cherry seed,ginseng;Chinese yam,dogwood;red bean,plantago seed;chrysanthemum,medlar;Chinese yam,Rehmannia glutinosa;Astragalus,Angelica;Coix seed,Cangzhu;Dangshen,Salvia miltiorrhiza.The 15 core medicine groups were drawn,namely: Angelica,Astragalus,Codonopsis,Chinese Yam,Cornus,Salvia;Poria(Poria cocos Bark);Medlar,Chrysanthemum,Achyranthes,Eucommia;Atractylodes,Coix Seed,Shiwei;Papaya,Wood Incense,cinnamon(cinnamon sticks);French pinellia,Citrus aurantium,scutellaria;Plantago,Chixiaodou,Polyporus;Golden cherry seeds,Gorgon;Bran-fried Atractylodes(Atractylodes);dried tangerine peel,motherwort,ginger peel;ginger pinellia;parsnips,Forsythia;reed root;Sichuan achyranthes bidentata,epimedium;wild jujube seed.Conclusions: 1.Instructor,Chief Physician Shi Junhua,the core drug efficacy for the treatment of primary nephrotic syndrome is to invigorate the spleen and nourish qi,invigorate the kidney and astringent,diure and reduce swelling,regulate qi and promote blood circulation,and clear heat and diuresis,reflecting the weakness of the spleen and kidney,removing water dampness and blood stasis,qi stagnation,phlegm heat and other evil qi,strengthening the body and eliminating evil,and treating both symptoms and root causes.2.From the point of view of drug nature and flavor,in clinical practice,patients with primary nephrotic syndrome are more inclined to cold constitution,which is consistent with the key pathogenesis of spleen and kidney weakness.From the perspective of drug return,the five internal organs of the spleen,lung,liver,kidney,and heart are in the same tune,unobstructed and harmonized,and the spleen,lung,liver,and kidney are the main components,so that the moisture evil can be transferred,reduced,relieved,and evaporated.,In order to achieve the power of strengthening the righteousness and eliminating evil.From the point of view of the efficacy of the medicine,both invigorating the deficiency and reducing the actual effect,the blood and yin and yang are in the same regulation,so that the body is coordinated and unobstructed.3.In the treatment of primary nephrotic syndrome,Chief Physician Shi Junhua strives to "the cure must be based on the foundation" and go directly to the hospital.Based on the key pathogenesis of "weakness of the spleen and kidney",the treatment is based on the "spleen",the "acquired" is used to supplement the "congenital",and "cultivating soil to restraint water." At the same time treat both symptoms and root causes,invigorate qi,nourish blood,invigorate the spleen,nourish the kidney and relieve essence,or apply diuresis,distend dampness and swelling,nourish the liver and calm the liver,soothe the nerves,dry dampness and diuresis,warm the kidney to help yang and qi,clear heat,resolve phlegm and regulate qi.Curative methods such as dispelling wind,heat dissipation and detoxification.
