Objective:1.Cross-sectional investigation of knowledge attitude and practice of oxygen therapy for nurses in emergency department of Shanxi,and to understand the current situation of clinical nursing oxygen therapy in emergency department of the city.2.To study the related factors affecting oxygen therapy knowledge and attitude of nurses in emergency department,to provide reference for the learning of oxygen therapy knowledge of emergency department nurses.Methods:1.In this study,the questionnaire survey was conducted to investigate 279 nurses of the emergency department of three Grade 3A General Hospitals and five Grade 2A General Hospitals in a certain city of Shanxi Province.The questionnaire adopted by the self-designed was oxygen therapy knowledge attitude and practice of nurses in emergency department.2.The statistical data were recorded with Excel 2010 and SPSS 22.0 for statistical analysis.(1)Mean±standard deviation or median(quartile range)was used to describe the knowledge and attitude scores of oxygen therapy.The practice of oxygen therapy was described by using frequency and proportion.(2)T test,analysis of variance and rank test were used to analyze the oxygen therapy knowledge and attitude score of different nurses were statistically significant.(3)Multiple linear regression were used to find out the influencing factors related to knowledge and attitude of oxygen therapy in emergency department.Results:1.Knowledge of oxygen therapy:(1)Oxygen therapy knowledge score was 10,the average score was 5.03 ±2.21.(2)There were statistical significance of oxygen therapy knowledge scores among nurses of different hospital grade,age,service year,professional titles,administrative posts,emergency specialized nurses(P<0.001).(3)The main factors influencing the score of oxygen therapy knowledge were professional titles,service year,hospital grade and age.(4)The nurses in the emergency department answered that the top three items of knowledge sources of oxygen therapy were work experience,and during the period ofschool education,continuing education,the proportion of them were 31.20% 20.31% and17.65% respectively.(5)The training and demand of oxygen therapy knowledge:in the past year,30.82% of the nurses who received the knowledge of oxygen therapy training accounted for 75.86% and 24.14% of the nurses who received training at the hospital and provincial level respectively.The number of nurses who had not been trained oxygen therapy in recent year accounted for 69.18 %,among them 1~2 years,2~3 years,3~4years,4 ~ 5years and before 5 years nurses who did not receive treatment knowledge training accounted for 14.4 %,16.4 %,9.6 %,16.0 %,43.6 % respectively.2.Attitude of oxygen therapy:(1)The full score of oxygen therapy attitude span was 7~35,and the average score was 21.84 ± 3.62.(2)There were statistical significance of oxygen therapy knowledge scores among nurses of different gender,age,service year,education,professional titles,administrative posts,emergency specialized nurses(P<0.001).(3)The main factors influencing the score of oxygen therapy attitude were age and administrative posts.3.Practice of oxygen therapy:(1)Investigation on oxygen therapy behavior of Nurses under the Model of "Independent-Cooperation" in Medical Care.(1)Investigation results of oxygen therapy behavior of nurses in emergency department follow the doctors' advice:51.97% of nurses said their department had a “target oxygen saturation” range for patients which were other than COPD or the risk of hypercapnia;20.79% of nurses indicated that their department doctor would set the range of “target oxygen saturation” in the oxygen therapy doctor's order for those patients.Among them,80.48%,9.26%,5.56%,3.70% of nurses described the setting target ranges were 90~95%,90~98%,90~100%,85~95% respectively.79.21%of nurses said that their department doctors did not set “target oxygen saturation” in patients' orders which were other than COPD or the risk of hypercapnia.Among them,82.67%,8.44%,6.22%,2.67% of the nurses believed that the “target oxygen saturation” of this type of patient was within the safe range of 90~95%,85~90%,90~100%,95 ~ 100% respectively.(2)Investigation results of independent oxygen therapy behavior of emergency nurses at work:88.17% of nurses monitored patients' oxygen intake.100% of nurses feedbacked the observed oxygen therapy to the doctor in time.94.98% of nurses recorded the patient's oxygen saturation and flow rate on the patient's monitoringrecord.93.55% of nurses gave different flow rate of oxygen when the patient used different ways of oxygen supply.19.35% of nurses used oxygen conveyors.91.40% of nurses increased oxygen concentration before sputum aspiration for patients undergoing mechanical ventilation.68.10% of nurses adjusted oxygen flow as needed in the process of inhaling oxygen.The first three reasons that required nurses to adjust oxygen flow in time were the sharp decreasing of blood oxygen saturation,When the patients are asked to do so,the proportion of them were 61.31%,34.16% and 2.94% respectively.(2)Nurse's oxygen therapy behavior under the mode of nurse-patient "instruction-cooperation" relationship:84.95% of nurses gave health education to patients with oxygen therapy knowledge.Among the methods adopted by nurses,46.35% of the nurses used pictures and texts to tell them on the spot,35.39% of the nurses issued paper versions for attention,9.36% of the nurses used other methods such as centralized simulated teaching and 8.90% of the nurses pushed them with WeChat Official account.About the results of the investigation on the compliance of patients with oxygen therapy:40.50% of nurses considered “good”,56.27% of nurses considered “general”,and3.23 % of nurses considered “poor”.The reasons for poor compliance with oxygen therapy in the first three places were:23.79% of the nurses thinked that oxygen inhalation was of little significance to the treatment of diseases,20.81% of the nurses thinked that inhalation of oxygen may cause pharynx discomfort,20.48% of the nurses thinked that was the cost.Conclusion:1.The nurses' knowledge and attitude of oxygen therapy were in the medium level in an emergency department of a certain city of Shanxi Province.The nurses' oxygen therapy behavior was more active,which needed to be further improved and promoted.2.Professional titles,service year,hospital grade and age were the main influencing factors of oxygen therapy knowledge scores.Age and administrative posts were the main influencing factors of oxygen therapy attitude scores.Nurses' oxygen therapy practice was good under the mode of "independent-cooperation",and further exploration on how to do standardized oxygen therapy under its mode can be regarded as the research direction of doctors and nurses in the future.
