Comparison Of Visual Quality After Implantation Of A1-UV And SN60WF Aspheric Intraocular Lens

Purpose: To evaluate the visual quality of eyes implanted with two aspheric intraocular lenses(IOL),domestic A1-UV IOL and American SN60 WF IOL with the same spherical aberration;to study the correlation between pupil diameters and the OQAS parameters;to evaluate whether the visual quality of domestic A1-UV IOL is analogous to that of American SN60 WF IOL and whether the intraocular SA of A1-UV IOL consistent with its design,so as to provide a basis for clinical practice.Methods: This was a prospective nonrandomized controlled study.A total of 105 eyes from 90 age-related cataract patients who underwent phacoemulsification and IOL implantation were enrolled between September 2017 to October 2018.The patients were divided into SN60WF(Alcon)group and A1-UV IOL(Eyebright)group.Finally,82 cases(96 eyes),42 cases(49 eyes)in SN60 WF group and 40 cases(47 eyes)in A1-UV group,were included in the present study.At three months postoperatively,UCVA and BCVA were recorded by logMAR;the contrast sensitivity tester(CSV-1000,Vector-Vision,USA)was used to measure for spatial frequencies 3cpd,6cpd,12 cpd,18cpd;corneal,intraocular and ocular total high order aberration(tHOA),spherical aberrations(SA),Trefoil and Coma were obtained by aberrometer(KR-1W,Topcon,Japan)under 4mm and 6mm pupillary diameters;objective scatter index(OSI),modulation transfer function cut off(MTF cut off),strehl ratio(SR),and contrast visual acuity OV at 100%,20%,and 9% were measured by Objective Quality Analysis System II(OQAS II,Visiometrics,Spain)under 2mm,4mm and 6mm pupillary diameters.Statistical analysis was performed with the independent samples t test.Spearman correlation analysis was used to analyze the correlation between the OQAS parameters and pupil diameters.Results: Three months postoperatively,There was no significant difference in UCVA and BCVA between the two groups(P=0.75,P=0.35).There was no significant difference in CS between the two groups at 3 cpd,6 cpd,12 cpd and 18 cpd(P=0.85,P=0.97,P=0.89,P=0.43).There was no significant difference in corneal SA at 6mm pupillary diameter between the two groups before and after operation(P=0.32,P=0.13).There were no significant differences in corneal tHOA,trefoil,coma and SA at 4mm and 6mm pupillary diameters between the two groups(P=0.12,P=0.87,P=0.36,P=0.69;P=0.15,P=0.77,P=0.94,P=0.13).There were no significant differences in internal tHOA,trefoil,coma and SA at 4mm and 6mm pupillary diameters between the two groups(P=0.07,P=0.50,P=0.24,P=0.26;P=0.11,P=0.50,P=0.82,P=0.13).There were no significant differences in ocular tHOA,trefoil,coma and SA at 4mm and 6mm pupillary diameters between the two groups(P=0.18,P=0.26,P=0.95,P=0.58;P=0.71,P=0.55,P=0.67,P=0.63).There was no significant difference between the actual measured SA-0.19 +0.05?m and the designed SA-0.20?m of A1-UV(P=0.91).There was no significant difference in OSI between the two groups(P=0.93).For MTF cut off,SR,OV100%,OV20% and OV9% at 2mm,4mm and 6mm pupillary diameters,no significant differences were found between the two groups(P=0.88,P=0.42,P=0.68,P=0.79,P=0.58;P=0.17,P=0.97,P=0.11,P=0.26,P=0.53;P=0.56,P=0.92,P=0.72,P=0.17,P=0.91).MTF cut off,SR and OV of the two groups were significantly negatively correlated with pupil diameters(P < 0.01).Conclusion: Subjective and objective visual quality of domestic A1-UV IOL is comparable to that of American SN60 WF.Postoperatively,the intraocular SA of A1-UV IOL is close to the design value and the ocular SA coincides with its design.A1-UV IOL as a domestic IOL is worthy of clinical promotion and application.Several parameters MTF cut off,SR and OV in pseudophakic eyes decreased with the increase of pupil diameters,which suggested that visual quality decreased with the increase of pupil diameters.
