Objective The objective is to observe the visual quality of traumatic cataract patients with aspheric multifocal intraocular lens.Methods The study selected30traumatic cataract patients (30eyes) in our hospital as the sample (27males,3females);.All the patients were between18-56years old, in which17patients (17eyes) troubled with corneal perforating,9patients (9eyes) were sufered from blunt trauma and4patients(4eyes) hurted by intraocular foreign body.Also sampled30simple cataract patients(30eyes).The inclusion criteria:Patients were between20~60years old; the astigmatism were within the bounds of-1.5D-1.5D; The corneal scar or wound was in peripheral zone; the densities of corneal endothelial cells were upon1000/mm2; there were displaces or deformations in pupil, especially in large pupils; The patient had the entire posterior capsular or lighter damaged on it which have no need to implant ciliary groove sutured intraocular lens; and there was no complication occurred pre-operative or post-operative. The exclusion criteria:There was corneal scra in the central or large wound after healing which make the visual acuity become worse; Patients who have diseases preoperative can make obvious influences on the function of fundus, such as vitreous foreign body, retinal tear and optic nerve-retinal contusion. And patients with a history of glaucoma, uveitis, retinal detachment, diabetes or autoimmune diseases were excluded. The30traumatic cataract patients(30eyes) were treated with implation of aspheric multifocal intraocular lens(Tecnis ZMA00) as the test group,and the30simple cataract patients(30eyes) were treated with implation of the same aspheric multifocal intraocular lens(Tecnis ZMA00) as the contrast group. All the operations about the study were performed by the same skilled doctor successfully with no complication happened postoperative. Uncorrected visual acuity(UCVA), best corrected visual acuity(BCVA), contrast sensitivity (CS) and stereoscopic vision were observed after the operation1month,3months and6months, at the same time, we obtained the satisfaction of patients by questionnaire survey.All the datas were analyzed by the SPSS17.0statistic software. Two way independent T-test was used to analyze the general condition preoperative. Wilcoxon Test was used to analyze the contrast sensitivity (CS) and stereoscopic vision. Take P<0.05as thesignificant level, means the difference was statistically significant.Results The UCVA and BCVA of test group were lower than the contrast group,but there were no statistically significant differences between the two groups1month,3months and6months postoperatively (P>0.05). And there were no statistically significant differences in stereoscopic and in every stages of contrast sensitivity (P<0.05). The questionnaire survey showed that there were30patients satisfied in percentage of100%after implantation of aspheric multifocal intraocular lens in the contrast group and there were30satisfied patients in percentage of100%in test group.Conclusions1.There was no differences between the visual quality after implantation of aspheric multifocal intraocular lens in traumatic cataract patients and in the simple cataract patients.2. Aspheric multifocal intraocular lens were more appropriate for the patients with small corneal scar, little damage in posterior capsule or better fundus function relatively. The reason may be it can reserve or even enhance the visual quality and the quality of life after the trauma.But there was a obvious glare phenomenon in the night, thus it is not applicable for the professional drivers, espically for the patients who usually drive in the night.
