Objective:to analyze and discuss the clinical effect of optimal use of tourniquet combined with danshen huoxue decoction on accelerating rehabilitation after total knee joint replacement through clinical comparison and observation,and to evaluate its safety.Methods:Choose from July 2016 to April 2017 in Jiangsu province hospital orthopedics accept unilateral knee arthroplasty 60 patients as the research object,according to random number table method of 30 cases were divided into observation group and control group 30 cases,control group patients who use a tourniquet,observation group is optimized using a tourniquet(only on the stage using a tourniquet prosthesis),combined application of salvia miltiorrhiza huoxue soup,compare the change of the observation group and control group cases relevant indicators,Items for comparison included blood cell analysis,perioperative blood loss,thigh circumference,postoperative VAS pain score,postoperative recovery of knee joint function,changes in intraoperative drainage volume,and postoperative incidence of deep vein thrombosis.Results:the hemoglobin count of the observation group on the third day after surgery was lower than that of the control group(P<0.05),and there was no significant difference between the two groups on the seventh day after surgery(P>0.05).The d-dimer difference between the two groups on the fifth day after surgery was significantly lower in the observation group than in the control group(P<0.05).The degree of postoperative lower limb swelling and the rate of detumescence in the observation group were all better than those in the control group,with statistical difference(P<0.05).The intraoperative dominant blood loss in the observation group was higher than that in the control group,and the postoperative drainage and recessive blood loss were lower than that in the control group.There were statistical differences between the two groups(P<0.05).However,there was no statistical difference in the total amount of perioperative bleeding(P>0.05).The VAS score of the observation group was lower than that of the control group,and the ROM of the knee joint on the third and seventh days after the surgery of the affected limb was significantly better than that of the control group(P<0.05).Ultrasound examination of lower limb blood vessels 2 days after surgery and 7 days after surgery showed that DVT did not occur,and there was no statistically significant difference between the observation group and the control group.Conclusion:the optimal use of tourniquet during TKA surgery does not increase the amount of perioperative bleeding,reduces postoperative pain,speeds up the detumescence of the affected limb,and promotes the early postoperative functional rehabilitation of patients,which has good safety and is worthy of clinical application.
