Objective:Observe the clinical efficacy of gu shang fu yuan soup Modified on the hidden blood loss after total knee arthroplasty,and anemia in patients with knee joint function with the relationship to improve the situation.Methods:Between June 2015 and June 2016,60 patients who underwent TKA because of osteoarthritis from Wendeng Orthopaedic Hospital of Shandong Province Hospital were enrolled in the study.All patients were randomly divided into two groups:experimental group and control group with 30 patients each.Control group:The tranexamic acid and sodium chloride injection(100ml:tranexamic acid 0.5g)would be used at a speed of 40-60drops/min,dosage:0.01g/kg was injected 30 minutes before operation,and the patients would received lg of tranexamic acid dissolved in 50 ml of normal saline were administered intra-articular injections after closed incision,Loosen the clip 2 hours later,Pull out after 24 hours.Experimental group:Do as the control group during the operation,however,preoperative treatment group taking three conventional Modified gu shang fu yuan soup,after beginning the first day of medication,treatment 2 weeks.Before the surgery and postoperative1?3?10days after operation in patients with venous blood samples for routine examination,hemoglobin,hematocrit,such as a comparison target;Before the surgery and postoperative24 hour after operation in patients with venous blood samples for routine examination,fibrinogn?prothrombin time?activated partial thromboplastin time and D-dimer;The visual analogue scale was used to assess the vas value of the knee resting state at the Preoperative and postoperative1?3?7,10 days after operation;Record the knee joint activity of preoperative,after 1 weeks,1 months,3 months of the knee joint function;HSS scoring system to assess the use of preoperative,after 1 weeks,1 months,3 months of the knee joint function.Results:Experimental group of hemoglobin and hematocrit values measured after administration were higher than,the difference was significant(P<0.05);Experimental group and the intraoperative blood loss was(116.8±25.30)ml and(109.66±34.90)ml,The experimental group and control group was no difference of coagulation test,the difference was not significant(P>0.05);the postoperative drainage volume was(260.09±43.61)ml and(314.29±40.77)ml,the difference was significant(P<0.05);the hidden blood loss was(509.68±170.63)ml,and control group after TKA hidden blood loss(596.78±98.65)ml,the difference was significant(P<0.05);eperimental group were 2 patients(7.6%)received a transfusion,control group were 5patients(20.8%)received a transfusion,the difference was not significant;The experimental group was better than the control group for D-dinner,the difference was significant(P<0.05);The experimental group was better than the control group in relieving pain by VAS,the difference was significant(P<0.05);The knee joint activity of the patients in the experimental group was better than that in the control group,the difference was significant(P<0.05);The HSS scores of the patients in the experimental group was better than that in the control group,the difference was significant(P<0.05).Conclusion:Modified gu shang fu yuan soup could reduce hidden blood loss after primary unilateral total arthroplasty,and treatment of patients with anemia status,and is conducive to the joint in patients with recent functional recovery,but long-term improvement in knee function had no significant effect.
