| Objective: After Compare in TKA Limited Use Tourniquet and Full use Tourniquet For postoperative Rapid recovery of Effect.Method: Total included 60 cases from december 2018 to december 2020,the department of orthopaedics,Affiliated Hospital of Chengdu University of Traditional Chinese Medicine Admitting 60 patients.Randomization Group A(Limited tourniquet use group):From start prior to osteotomy tourniquet inflation up to prosthesis Installation cement hardening post release tourniquet Group B(Full tourniquet group):From start before skin incision tourniquet inflation to suturing of incision completed compression bandaging the tourniquet was then released.By collection Patients in both groups of the operation time tourniquet time intraoperative blood loss,Post-op blood loss,Pre-Post-op VAS score,Pre-Post-op ROM,Pre-Post-op HSS score,and C-reactive protein,Serum creatine kinase,swelling degree of operative limb,postoperative wound complications analyze and compare.Result: After last result analysis,We see that group B in Intraoperative blood loss Significantly less group A.In total postoperative blood loss,Occult blood loss,Drain volume more group B than group A.Tourniquet in Group A usage time It was significantly lower than that in group B.Patients in arm A early postoperative VAS score significantly lower than Group B,respectively after surgery 1 and 3 days after surgery.Postoperative HSS score A after surgery significantly higher than group B on Day 3.Group A postoperative range of motion significantly higher than Group B Respectively appear in fig 1 and 3 days after surgery.In both groups,the operation time,postoperative hemoglobin on other indicators of performance was not significantly different.Conclusion: Full tourniquet group not effective effective reduction total postoperative blood loss,and compared to limited tourniquet use with Post-Op early pain early mobilization degree worse higher postoperative Postoperative complications occurrence.Therefore,with less tourniquet time can be significantly reduced TKA Post-related complications,and can be improved knee ROM,favors improvement after replacement Satisfaction. |
