ObjectiveTo investigate the total knee arthroplasty (TKA) in the use of tourniquet different impact on the amount of bleeding and early clinical effectMaterials and MethodsJune2010to June2011,200patients with unilateral TKA were randomized into three groups, each of50cases. A group:the beginning of preoperative use of a tourniquet, tourniquet release after joint replacement complete hemostasis, suture wound closure elastic bandage;B group:surgery beginning to the end of the full use of a tourniquet, cut off after the release of elastic bandage tourniquet; C group: preoperative use of a tourniquet, revealed after this osteotomy before the tourniquet, elastic bandage to close the incision after the release of the tourniquet. Recorded blood loss, postoperative drainage, operative time, tourniquet time, blood transfusion cases, calculating the amount of hidden blood loss and the total amount of bleeding in each group were compared.Results:A group and B group, the total blood loss was no significant difference (P>0.05), C group and A, B group differences were statistically significant (P<0.05); A group of operative time was (97.7±14.6) min, B group (93.6±17.5) min, C group (98.9±18.5) min, no difference among the three groups Statistically significant (P> 0.05). A group of tourniquet time was (79.4±14.4) min, B group (93.6±17.5) min, C group (60.9±12.5) min, compared among the three groups There were significant differences (P<0.05).the incidence venous thrombosis of C group and wound of C group healing were all lower than A and B group.the HSS score of three groups have no obvious difference.ConclusionUse of a tourniquet to start after osteotomy to release the tourniquet after joint replacement complete hemostasis, suture wound closure elastic bandage to reduce the total surgical blood loss and the incidence of complications after operation.
