BackgroundCI accounts for 70% of all acute cerebrovascular diseases.Cognitive impairment after cerebral infarction,as a common complication of cerebral infarction,is also a common type of VCI,which brings great distress and burden to patients,families and society.However,in terms of treatment,cognitive impairment after cerebral infarction is superior to AD by controlling risk factors and protecting cerebral vessels to a certain extent to prevent its occurrence or delay its progress.Therefore,the metabolic index which can predict its occurrence in the blood has become the focus of scientific research,among which leptin,as a possible index for clinical application,has been paid more and more attention.Leptin,as a kind of protein,produced from fat.It regulates energy metabolism and cognitive function.On the one hand,the concentration of leptin in peripheral blood is related to body fat content,which may indicate the high risk of cerebrovascular disease.On the other hand,leptin can improve cognitive function by enhancing synaptic signal transduction,regulating neuronal development and apoptosis,and reducing phosphorylation of a ? and Tau protein inside and outside cells.Therefore,the study of the relationship between leptin and VCI is of greatsignificance in assessing the risk of post-stroke cognitive impairment and early intervention,and may even improve its prognosis.Previous studies on leptin have mostly studied the direct effect of leptin on cognitive impairment in animal experiments in vitro,but few studies have been done on leptin level in human peripheral blood serum.In this study,we compared the levels of leptin in peripheral blood of patients with and without cognitive impairment after cerebral infarction to explore the effect of leptin on cognitive impairment.ObjectiveIn this study,we compared the levels of leptin in peripheral blood of patients with and without cognitive impairment after cerebral infarction to explore the effect of leptin on cognitive impairment,and to explore the correlation between leptin level and the degree of cognitive impairment.To analyze the relationship between leptin and blood lipid,GHb,BMI,to explore the relationship between leptin and common risk factors of cerebrovascular disease.Methods:The subjects in this study were divided into three groups: cognitive normal group(n = 60)with new cerebral infarction without cognitive impairment(n = 60),cognitive disorder group(n = 60)with new cerebral infarction with cognitive impairment(n = 60),blank control group for the same period of physical examination of 60 healthy middle-aged andelderly.In blank control group,4 ml of peripheral blood was taken on an empty stomach,and 4 ml of peripheral blood was taken on an empty stomach in the cognitive normal group and the cognitive impairment group within 48 hours of the onset.The plasma leptin level was measured by ELASA,then we research the relationship among the three groups was compared.In addition,the relationship between leptin and TG,BMI,GHb,TCHO was compared.Results:1.There was no statistical difference in MoCA score between normal cognitive group and normal cognitive group(p > 0.05),but there was significant difference in MoCA score between normal cognitive group and cognitive impairment group(p < 0.01).2.The peripheral serum leptin of patients with cognitive impairment was the lowest among the three groups,and there was statistical difference between cognitive impairment group and cognitive normal with cerebral infarction group(p < 0.05),and there was no statistical difference between cognitive normal with cerebral infarction group and the blank control group(P>0.05).3.There was a significant negative correlation between serum leptin level and cognitive impairment(p < 0.01,r = 0.528).4.There was a correlation between serum leptin level and BMI(p=0.016 < 0.05),and a positive correlation was found between thecorrelation coefficient(r = 0.310 > 0)and the correlation coefficient.5.There was a correlation between serum leptin level and TG(P=0.02<0.05),and a positive correlation was found between serum leptin level and serum leptin level(r = 0.398 > 0).6.There was no significant correlation between serum leptin level and TCHO(p = 0.303 > 0.05).7.There was no significant correlation between serum leptin level and GHB(p =0.954 > 0.05).Conclusions:1.The patients with cerebral infarction with cognitive impairment were lower than those without cognitive impairment.2.Leptin level was negatively correlated with cognitive impairment.3.Leptin level was positively correlated with BMI,TG.
