Background:Cerebral infarctions often can cause Vascular cognitive impairment no dementia (VCIND).The symptoms of VCIND are relatively unconspicuous, so it is not an easy work for clinicians to identify VCIND. Following the progression of the disease, a large proportion of VCIND will develop into Vascular dementia (VD), which can cause irreversible cerebral damages.In the early stage of VCIND there are many risk factors which can be intervened, so if discovering VCIND at it's early stage and giving intervention at this stage will be possible to delay or prevent the development of VD.So correctly diagnosing VCIND at it's early stage will undoubtedly have great preventive significance. Diagnosing VCIND in post-stroke patients need collect detailed clinical, neuropsychological and imaging materials.The imaging characters of VCIND are various.There are lots researches about imaging characters of VCIND in China and abroad recently. However, there are no accepted imaging criteria right now, it still need further researches.Objective:Study of risk factors of the patients with VCIND a fter cerebral infarction.In order to provide evidence for early diagn osis, implementing intervention.Methods.The sample population is divided into two groups:VCIND after cerebral infarction group (VCIND,100);Cognitive normal after cerebral infarction group (NoCI,110).All subjects accept neuropsycholo-gical testing, brain MRI testing and their detailed materials about demology and clinical history are collected. In this research, we will explore the relationship in the following aspects:Analysising the brain magnetic resonance imaging (MRI) of VCIND and NoCI patients, to study the relationship between the imaging damages and cognitive impairment. The imaging damage including the the following aspects:the location, amount and size of cerebral infarction region; The location and degree of the white matter lesion (WML);Linear measurement of the cerebral atrophy. Result: The first:Results of single factor analysis:1.â‘ In the VCIND group,the incidence of infarction in left hemisphere, frontal and temporal lobe, anterior limb of internal capsule, genu of internal capsule and thalamus are higher than in the NoCI group, there are statistical differences.â‘¡In the VCIND group,the incidence of having more than 3 infarction regions are higer than in the NoCI group,there is statistical difference.â‘¢The incidence of medium size of cerebral infarcts in the VCIND group are significantly higher than of the NoCI group,there is statistical difference.2.In the cerebral infarcts of the VCIND group, it is more common to see cerebral infarctions accompany with leukoaraiosis than the NoCI group.The incidence of leukoaraiosis in the centrum semiovale in the VCIND group are higher than in the NoCI group.The incidence of degree 3 leukoaraiosis in the VCIND group are higher than in the NoCI group, there is statistical difference.3.There are no statistical difference in Linearity measurement of brain atrophy between the two groups.The second:Results of non-conditional Logistic regression analysis:The location of cerebral infarction, the amount and size of infarction regions, the degree of the leukoaraiosis after cerebral infarction are closely related with VCIND. Cerebral infarction in frontal and temporal lobe, anterior limb of internal capsule, genu of internal capsule,thalamus and the leukoaraiosis in centrum semiovale after cerebral infarction are the risk factors of the VCIND.Conclusion:1.The location of cerebral infarction, the amount and size of infarction regions are closely related with VCIND.Infarctions in the frontal and temporal lobe, anterior limb of internal capsule,genu of internal capsule and thalamus are the risk factors of the VCIND. 2.The degree of the leukoaraiosis is closely related with VCIND after cerebral infarction. The leukoaraiosis in centrum semiovale are the risk factors of VCIND after cerebral infarction.
